국문 초록

본 연구는 2018–2023년 국민건강보험 청구자료와 국가건강검진 자료를 연계한 패널을 활용해 연령, 질병군, 서비스유형별 지출 변화를 분석하였다(모든 금액은 2023년 불변가 기준). 패널 고정효과 분석 결과, 65세 이상 인구 비율이 1%p 증가할 때 실질 총지출은 0.82%(β=0.0082, p<.01) 상승했으며, 입원(0.45%p)과 외래(0.29%p) 지출 증가가 두드러졌다. 장기요양 확대와 치매국가책임제 시행 이후 고령층 1인당 연간 실질의료비는 비고령층보다 38만 원 높았고, 지출 집중도(지니계수 0.438) 또한 유의하게 높았다. 분위 및 로지스틱 분석에서는 상위 10% 고비용군의 확대와 만성 다질환, 비수도권 거주와의 상호작용이 나타났다. 이러한 결과는 고령화가 총지출 증가와 분포 불균형을 동시에 심화시킴을 보여주며, 고비용 환자 집중관리, 예방 및 만성관리 강화, 서비스 구조 개편, 재정 건전성 확보, 지역 격차 완화의 필요성을 시사한다. 비급여·간병비 제외와 인과추론 한계가 존재하며, 향후 자연실험 기반 연구가 필요하다.

영문 초록

This study analyzed changes in healthcare expenditures by age, disease group, and service type using an anonymized panel linking National Health Insurance claims and national health screening data from 2018–2023 (all values in 2023 constant prices). In a panel fixed-effects model, a 1%p increase in the 65+ population was associated with a 0.82% rise in real total expenditure (β=0.0082, p<.01), driven by inpatient (0.45%p) and outpatient (0.29%p) services. Following the expansion of long-term care and the National Dementia Responsibility Program, annual per-capita spending among older adults was 380,000 KRW higher than among younger adults, with greater expenditure concentration (Gini=0.438). Quantile and logistic analyses indicated growth of the top 10% high-cost group and interactions with multimorbidity and non-metropolitan residence. These results imply the need for targeted management of high-cost patients, stronger preventive and chronic care, service delivery restructuring, fiscal sustainability measures, and regional equity improvements. Limitations include the exclusion of non-covered and caregiving costs and constraints on causal inference; future work should leverage natural experiments.
