청년 건강 증진을 위한 미국 정책 사례 및 시계열 자료 분석

국문 초록

본 연구는 UN의 지속가능발전 세 번째 목표가 주창하는 ‘모두의 건강한 삶 보장(Healthy life for all)’의 관점에서 미국 연방 및 주정부의 청년 건강정책 사례와 실태를 분석하고 한국과 국제사회에 주는 정책적 시사점을 도출하고자 하였다. 이를 위해 미국의 ACA, Healthy People 2030, WSCC 사례를 중심으로 정책 구조와 성과를 고찰한 결과 청년층의 건강을 생애주기적 관점에서 통합적으로 관리하고 예방하는 체계를 확인할 수 있었다. 아울러 미국 청년 건강정책의 주요 대상인 12세-17세와 18-34세 두 연령집단의 건강권 보장(무보험자 비율)과 정신건강(우울감 또는 불안정서 지속 경험자 비율)의 추이를 파악하기 위해 미국 질병관리본부(NHIS)의 최근 4개년 자료(2019년~2023년)와 아동가족통계포럼의 최근 17개년 자료(2004-2021년)를 시계열 분석하였다. 분석 결과, 무보험자 비율은 전반적으로 감소 추세를 보인 반면 2020년 이후 우울감을 겪고 있는 청소년과 불안증을 겪는 청년층의 비율은 급격히 증가하여 코로나19 이후 청년층의 정신건강 악화 가능성을 시사하였다. 전술한 결과로부터 본 연구는 한국 사회 또한 건강과 웰빙을 포괄하는 방향으로 청년정책 설계 및 방향을 전환해야 할 당위성을 논의하였다. 나아가 국제사회가 주창하는 건강권 보장의 차원에서 청소년기부터 시작되는 누적적 불이익을 사전 예방하기 위한 제도적 개입과 증거기반 정책수립의 필요성을 촉구하였다.

영문 초록

This research aims to explore the US trends of youth health indicators and relevant policy cases within the Universal Health Coverage (UHC) perspective under Goal 3 in the United Nations Sustainable Development Goals (SDGs). I find that key policies reflect an integrated, life course-based approach to youth health and well-being. The findings underscore the need for South Korea to move beyond employment-and income-centered youth policies and to adopt a broader framework embracing health and well-being, with early preventive intervention targeting disadvantaged group. Using the US federal data from 2004 to 2023, I conducted the time-series analyses for the uninsured rate and mental health indicators among the youth population. Results show that a clear decline in the uninsured rate but a simultaneous deterioration in terms of anxiety and depression. These empirical results indicate that youth mental health issues are influenced by structural vulnerabilities and limitations in existing social protection mechanisms, rather than being temporary crises. In conclusion, this study discusses policy implications for youth health rights in both domestic and international context, drawing on lessons from the US case.
