국문 초록

본 연구는 병원 내 심정지 상황에서 간호사의 전문심장소생술(Advanced Cardiovascular Life Support, ACLS) 수행 의도를 조사하고, 계획된 행위이론(Theory of Planned Behavior, TPB)에 기반을 두어 관련 요인을 확인하고자 수행되었다. 상급종합병원에 근무하며 임상경력 6개월 이상인 간호사를 대상으로 한 서술적 횡단연구로, 연구자가 개발한 구조화된 설문지를 이용하여 ACLS 수행의도, 태도, 주관적 규범, 지각된 행위통제에 대한 자료를 수집하였다. 수집된 자료는 기술통계, Mann–Whitney U 검정, χ²검정, 로지스틱 회귀분석을 통해 분석하였다. 총 304명의 참여자 중 97.4%는 여성이었고 50.3%는 20대였으며, 일반병동근무간호사는 58.2%, 평균임상경력은 89.7개월로 나타났다. CPR 경험이 있는 간호사는 78.6%, 유효한 CPR 자격증을 보유한 간호사는 66.1%였으며, 7점 척도 기준으로 수행 의도(5.8±1.0), 태도(5.7±1.0), 주관적 규범(5.1±1.1), 지각된 행위통제(4.6±1.1)의 평균점수가 확인되었다. 로지스틱 회귀분석결과, 태도(aOR=5.02, 95% CI:2.03–10.94, p<.001)와 주관적 규범(aOR=4.35, 95% CI:1.79–10.57, p=.001)이 ACLS 수행 의도의 유의한 예측요인으로 나타났다. 이에 따라 간호사의 ACLS 수행역량강화를 위해서는 필수교육이수뿐 아니라 긍정적인 태도 형성과 사회적 지지를 높일 수 있는 기관 차원의 전략이 요구되며, 나아가 전문직 간 협업 기반의 교육을 활성화하여 ACLS 관련 수행 의도를 증진시킬 필요가 있다.

영문 초록

This study aimed to examine nurses’ intentions to perform advanced cardiovascular life support (ACLS) during in-hospital cardiac arrest and to identify the associated factors based on the theory of planned behavior. A descriptive cross-sectional design was employed, targeting nurses with more than six months of clinical experience at a tertiary hospital. Data were collected using a researcher-developed structured questionnaire that measured ACLS-related intention, attitude, subjective norms, and perceived behavioral control. The collected data were analyzed using descriptive statistics, the Mann–Whitney U test, χ² test, and logistic regression analysis. Among the 304 participants, 97.4% were female, and 50.3% were in their 20s. Nurses working in general wards accounted for 58.2%, with an average clinical experience of 89.7 months. In total, 78.6% had prior CPR experience, and 66.1% possessed valid CPR certifications. On a 7-point scale, the mean scores for intention, attitude, subjective norms, and perceived behavioral control were 5.8±1.0, 5.7±1.0, 5.1±1.1, and 4.6±1.1, respectively. Logistic regression analysis indicated that attitude (aOR=5.02, 95% CI:2.03–10.94, p<.001) and subjective norms (aOR=4.35, 95% CI:1.79–10.57, p=.001) were significant predictors of ACLS intention. These findings suggest that to enhance nurses’ competency in ACLS, it is essential to implement mandatory training programs and institutional strategies that cultivate positive attitudes and strengthen social support. Additionally, promoting inter professional education may further reinforce nurses’ behavioral intention to perform ACLS in clinical settings.
