알코올 사용장애 예측요인에 대한 국내 연구의 체계적 문헌고찰

국문 초록

본 연구는 국내에서 수행된 알코올 사용장애 관련 연구 중 2015년부터 2023년까지 발표된 학술논문 29편을 체계적 문헌고찰(Systematic Literature Review) 방법으로 분석하였다. 주요 예측요인을 정신·심리적, 신체적, 사회·환경적, 가족 요인으로 분류하고, 연구 시기에 따른 동향 변화를 비교·검토하였다. 그 결과 초기 연구들은 우울, 스트레스, 자아존중감 등 심리적 요인에 초점을 맞춘 경향이 강했으나, 최근에는 사회적 낙인감, 경제적 박탈, 부모화, 직무스트레스 등 사회․환경적 요인의 영향에 대한 분석이 확대되었다. 특히 Bronfenbrenner의 생태체계 이론에 기반하여 사회적 맥락 속 개인의 적응력과 음주 행동 간의 연관성을 통합적으로 해석하려는 시도가 나타났다. 본 연구는 기존 문헌고찰에서 다루지 않았던 미시적 요인들을 새롭게 반영하고, 예측요인 간 상호작용을 다차원적으로 정리함으로써 알코올 사용장애 예방 및 개입 전략 수립에 실질적 기초자료를 제공하고자 한다.

영문 초록

This study conducted a systematic literature review of 29 peer-reviewed Korean academic articles published between 2015 and 2023, with the aim of identifying key predictive factors for alcohol use disorder (AUD). The selected studies were categorized based on four domains: psychological, physical, social-environmental, and familial factors. A comparative analysis by period revealed that earlier studies primarily focused on individual psychological characteristics such as depression and stress, whereas more recent studies have increasingly addressed micro-level social factors including social stigma, economic deprivation, parentification, and job stress. Drawing on Bronfenbrenner’s ecological systems theory, recent research has emphasized the importance of contextual factors and their influence on individuals’ adaptive capacity and drinking behavior. This review also highlights the growing attention to the interaction of multi-dimensional predictors, reflecting a shift toward a more integrated and structural understanding of AUD. By incorporating underexplored variables and broadening the scope of analysis, this study provides foundational knowledge for the development of effective prevention and intervention strategies tailored to the complex nature of alcohol use disorder.
