국내 의료기관의 환자안전사고 관련 사망 현황 분석

국문 초록

본 연구는 국내 의료기관의 환자안전사고 관련 사망사고 현황을 분석하여 추후 환자안전사고 관련 연구의 기초자료를 제공하기 위해 시도되었다. 의료기관인증평가원의 2023년 환자안전 통계 데이터 중에서 환자안전사고 11,039건을 분석 대상으로 하였다. 환자안전사고 대상자의 일반적 특성 및 의료기관의 특성 관련 빈도를 살펴보고, 일반적 특성 및 의료기관 특성에 따른 환자안전사고 관련 사망 현황을 분석하였다. 환자안전사고는 성별, 연령, 사고종류, 사고발생시간, 사고발견시간 및 사고보고자에 따라 차이가 있었으며 사망사고 관련하여 연령은 19세 이하에 비해 70세 이상 79세 미만이 4.693배, 사고 종류는 기타에 비해 식사 및 영양과 관련된 발생위험이 10.687배로 나타났으며 사고발생시간은 낮번에 비해 초번 근무가 1.707배, 사고발견시간은 낮번에 비해 밤번근무가 2.638배, 사고보고자는 전담인력에 비해 보건의료기관 장이 12.911배 높은 것으로 나타났다. 환자안전사고 관련 사망사고 예방을 위해 연령에 따른 식사 및 영양보조 시 간호인력 수준 재배치, 보건의료기관장의 체계적인 관리시스템 마련 등이 필요한 것으로 나타났다.

영문 초록

This study was conducted to analyse the current status of patient safety accidents and fatalities in Korean medical institutions to provide a basis for future research on patient safety accidents. Among the 2023 patient safety statistics data of the Korea Institute for Accreditation and Evaluation of Medical Institutions, 11,039 cases of patient safety accidents were analyzed. The frequency of the general characteristics of the subjects of patient safety accidents and the characteristics of medical institutions was examined, and the influence of the general characteristics and characteristics of medical institutions on the fatal accidents of patient safety accidents was confirmed. Patient safety accidents differed by gender, age, accident type, accident occurrence time, accident discovery time, and accident reporter. In terms of fatal accidents, the age was 4.693 times higher for those 70 to 79 years of age compared to those 19 years or younger, and the risk of accidents related to meals and nutrition was 10.687 times higher compared to other types of accidents. The time of accident occurrence was 1.707 times higher for first shifts compared to day shifts, the time of accident discovery was 2.638 times higher for night shifts compared to day shifts, and the person reporting the accident was 12.911 times higher for heads of healthcare institutions compared to dedicated personnel. It was found that urgent measures are needed to prevent fatal accidents related to patient safety accidents, such as reallocating the level of nursing staff for meals and nutrition according to age, and establishing a systematic management system for heads of healthcare institutions.
