정신장애인 지역사회전환시설의 성과 및 개선방안 연구

국문 초록

본 연구는 정신건강 정책에서 탈원화의 역할을 하는 지역사회전환시설의 기능, 성과 및 한계에 대한 분석을 통해 환경변화에 따른 운영체계 개선 방안에 대해 제언하고자 하였다. 이를 위해 지역사회전환시설 전체 7개 기관을 대상으로 입소자에 대한 양적, 질적 분석을 하였다. 2023년 1년간 입소자 487명에 대한 입소 전후의 변화를 양적으로 분석하였으며, 입소자, 가족 및 유관기관 실무자 총 14명을 대상으로 이용 경험에 대한 질적 분석을 하였다. 분석 결과 지역사회전환시설을 이용함으로써 정신과적 증상, 삶의 질, 일상생활 및 대인관계 기능에 유의미한 변화가 나타났다. 퇴소 후 1개월 시점 지역사회 거주율이 89%, 외래유지율이 93.7%로 국가정신건강 현황조사 결과보다 높게 나타났다. 질적 분석 결과 참여자들은 24시간 전문가의 개별화되고 밀착적인 서비스를 통해 약물증상관리가 안정적으로 이루어지고, 의식주, 건강, 관계, 일상활동, 지역사회 참여가 효과적으로 이루어짐으로써 삶을 회복해 가는 것으로 나타났다. 하지만 공간, 규칙, 접근성, 연계에서의 한계점도 지적되었다. 연구 결과를 바탕으로 지역사회전환시설의 기능을 ‘단기거주 집중 사례관리’로 명확히 하고, 이를 위한 시설, 인력, 서비스 체계, 산출 기준 및 운영체계 개선 방안을 제언하였다.

영문 초록

This study aims to propose improvements to the operational system of community transition facilities, which play a role in the deinstitutionalization process within mental health policy, by analyzing their functions, outcomes, and limitations in the context of changing policy environments. A comprehensive quantitative and qualitative analysis was conducted across all seven community transition facilities. Quantitative analysis examined changes among 487 residents over the course of 2023, assessing differences before and after facility admission. In addition, qualitative data were collected through in-depth interviews with 14 individuals, including residents, family members, and service providers, to explore user experiences. The results indicated significant improvements in psychiatric symptoms, quality of life, daily living skills, and interpersonal functioning following facility use. One month post-discharge, the community residence rate was 89%, and the outpatient service retention rate was 93.7%, both surpassing national mental health survey benchmarks. Qualitative findings revealed that participants experienced stabilization of symptom management and recovery of life functions—including housing, food, health, relationships, daily activities, and community participation—through individualized, intensive, 24-hour professional support. Nonetheless, limitations regarding facility space, operational rules, accessibility, and interagency linkages were also identified. Based on these findings, the study proposes that the role of community transition facilities be clearly defined as “short-term residential intensive case management” and offers recommendations for improving facility infrastructure, staffing, service delivery systems, performance standards, and overall operational frameworks.
