의약분업정책을 통한 다중흐름모형(MSF)의 검증 분석

국문 초록

본 연구의 목적은 시기별 의약분업정책에 원형모형인 다중흐름모형(MSF)을 적용한 후, 기존 MSF에 의약분업정책이 모두 부합될 경우 그대로 유지할 것을 제시하고, 새로운 변수가 도출될 경우 이를 반영한 수정모형을 조명하는 등 이론적 시사점을 고찰하고자 하는 것이다. 분석결과, 정책대안흐름인 김영삼대통령의 약사법 개정공포가 촉발기제가 되어 의약분업정책의 창을 열었으며, 이를 통해 시발기, 폭발기, 정착기에 거쳐 의약분업정책을 둘러싸고 이익집단인 약사회와 의사회 간 치열한 상호작용이 이어지게 된다. 결국, 김대중대통령은 대체조제 금지, 주사제의약분업 제외 등이 반영된 약사법 수정안을 재개정공포함으로써, 정책은 일단 마무리되어 창은 닫히게 된다. 이는 약사회에 대해서 위상쇠락을 가져온 반면, 의사회에 대해서는 위상상승으로 변동시킨 것이다. 이를 근거로 제 시사점을 도출할 수 있는데, 핵심적 시사점으로는 의약분업정책을 통해 MSF 대비 새롭게 발견된 이익집단 위상의 변동모형(ICF), 새로운 촉발기제로서 정책대안흐름, 정책중개자를 추가적으로 반영하여 MSICF을 수정모형으로 제시한 부분이다. 이러한 MSICF를 통해 연구의 확장성이 높은 수준이 되었고, 복잡한 정책행태를 보다 논리적으로 조명하고 있는 기대효과가 도출되었다는 점에서, MSF에 비해 차별성을 갖게 된 것이다. 이외에 주변적 시사점으로 복잡한 정책행태도 일정한 질서틀이 존재하고, 경계의 변수중복성이 나타났으며, 이익집단의 강한 반발은 위상상승으로 연계될 수 있다는 것 등이 그것이다.

영문 초록

The purpose of this study is to verify the Multiple Stream Framework(MSF) using the interaction between the Korean Pharmaceutical Association and Korean Medical Association surrounding the separation of dispensary from medical practice policy. According to the analysis, the triggering mechanism of the policy alternative stream has opened the window of the separation of dispensary from medical practice policy. In the window of policy, intense interactions between interest groups are continued by period. Eventually, president Kim, Dae-jung proclaimed amendments to the Pharmacopoeia Act that reflected the prohibition of alternative preparations and the exclusion of the division of injectable drugs, so that the policy was closed and the window was closed. This policy output has led to a decline in fortunes for the Korean Pharmaceutical Association, while fortunes rose for korean medical association. Implications as the basis of the foregoing analysis, I would like to present MSICF as a modified framework by additionally reflecting the Interest Group Standing Change Framework (ICF), policy alternative stream as a new trigger mechanism, policy broker as newly discovered compared to MSF through separation of dispensary from medical practice policy. In addition, as a peripheral implication, complex policy behaviors have a certain orderly framework, and variable redundancy of boundaries has appeared.
