회상중심 집단통합예술치료가 요양병원 입원 노인의 우울감과 자아존중감에 미치는 영향 : 사례연구

국문 초록

본 연구는 회상중심 집단통합예술치료가 요양병원에 입원한 노인의 우울감과 자아존중감에 미치는 영향을 살펴보기 위한 사례연구이다. 연구대상은 A시 소재 B요양병원에 입원한 노인 5명으로, 회상과 의사소통이 가능한 분들로 선정하었다. 본 프로그램은 주 1회, 총 12회기로 구성되었다. 측정 도구로는 한국형 노인우울검사(KGDS), 자아존중감 척도(RSES), 나무그림검사 (Baum Test)를 사용하였으며, 사전 사후 검사를 통해 양적 질적 변화를 종합적으로 분석하였다. 그 결과, 참여자의 우울 점수는 평균 2.6점 감소하였고(p = .098), 자아존중감 점수는 평균 2.2점 증가(p = .051)하여 통계적으로 유의한 변화를 나타냈다. 또한 회기별 분석에서는 초기의 소극적인 정서 표현이 중기 이후 점차 확장되고, 후기에는 자기수용과 감정표현이 활발해지는 변화를 보였다. 나무그림검사에서도 정서적 안정과 자기 개념의 긍정적 변화가 관찰되었다. 이러한 결과는 회상중심 집단통합예술치료가 요양병원 입원 노인의 우울감 감소와 자아존중감 향상에 실질적인 효과가 있음을 보여준다. 특히 요양병원이라는 폐쇄적이고 제한적인 환경 속에서도, 예술적 매체와 회상을 통합한 접근은 정서적 회복과 자기 긍정 경험을 촉진할 수 있다는 점에서 실천적 가치가 크다.

영문 초록

This study is a case study to examine the effect of recall-oriented group integrated art therapy on depression and self-esteem of the elderly admitted to a nursing hospital. The subjects of the study were 5 elderly people admitted to B nursing hospital located in A city, who could recall and communicate. This program consisted of 12 sessions once a week. The Korean-style elderly depression test (KGDS), self-esteem scale (RSES), and tree drawing test were used as measurement tools, and quantitative and qualitative changes were comprehensively analyzed through pre- and post-tests. As a result, the participant's depression score decreased by 2.6 points on average (p=.098) and the self-esteem score increased by 2.2 points on average (p=.051), showing a statistically significant change. In addition, in the analysis by session, the initial passive emotional expression gradually expanded after the middle period, and in the latter half, self-acceptance and emotional expression became active. In the tree picture test, positive changes in emotional stability and self-concept were also observed. These results show that recall-oriented group integrated art therapy has a practical effect on reducing depression and improving self-esteem of the elderly in nursing hospitals. In particular, even in the closed and limited environment of a nursing hospital, an approach that integrates artistic media and recollection has great practical value in that it can promote emotional recovery and self-positive experiences.
