해외 거주 한국 노인의 한국 의료서비스 이용 경험: 기대와 현실

국문 초록

본 연구는 해외 거주 한국 노인의 한국 의료서비스 이용 경험을 심층적으로 탐색하고, 이들이 기대하는 의료서비스와 실제 경험하는 현실 간의 차이를 분석하는 것을 목적으로 하였다. 많은 한국 노인이 해외에서 생활하면서도 한국 의료서비스를 선호하는 경향을 보인다. 이에 본 연구는 해외 거주 한국 노인이 한국 의료서비스를 이용하는 과정에서 겪는 주요 경험과 어려움을 분석하고자 하였다. 본 연구는 질적연구방법을 적용하여 심층적인 이해를 도모하였다. 미국에 거주하는 65세 이상의 한국 노인 12명을 대상으로 심층 면담하였으며, 의료서비스 이용 경험, 기대와 현실 간의 차이, 개선 필요성 등에 대한 생생한 경험을 수집하였다. 면담 자료는 주제 분석 방법을 활용하여 주요한 패턴과 주제를 도출하였다. 또한, 연구의 신뢰성과 타당성을 확보하기 위해 연구 참여자의 진술을 반복적으로 확인하고, 유사한 연구와의 비교를 통해 결과의 일관성을 검토하였다. 연구 결과는 해외 거주 한국 노인은 한국 의료서비스의 신속성과 높은 의료 수준을 기대하고 있었으며, 특히 해외 의료체계와 비교할 때 한국 의료진과 원활하게 소통할 수 있다는 점에서 심리적 안정감을 느끼는 것으로 나타났다. 그러나 현실적으로는 몇 가지 문제점이 확인되었는데 첫째, 국민건강보험 미적용으로 인해 의료서비스 이용 시 높은 경제적 부담이 발생하였다. 둘째, 한국 의료기관의 신속한 진료 시스템이 오히려 의료진과의 상담 시간을 제한하여 충분한 의사소통이 이루어지지 않는다는 불만이 제기되었다. 셋째, 미국 의료체계에서 경험한 환자 중심(patient-centered) 접근과 달리, 한국의 의료서비스는 여전히 의료진 중심으로 운영되는 경향이 강해 해외 거주 노인들에게 익숙하지 않은 방식으로 인식되었다. 본 연구 결과를 바탕으로 다음과 같은 개선 방안을 제안한다. (1) 일정 기간 해외 거주한 한인들에게 단기 체류 시 국민건강보험 가입을 허용하는 방안 검토, (2) 해외 거주 한국 노인을 위한 의료정보 제공 강화 및 온라인 플랫폼 구축, (3) 노인 친화적 의료서비스 도입. 이러한 정책적 개선이 이루어진다면, 해외 거주 한국 노인의 한국 의료서비스 이용 경험이 더욱 만족스러워질 뿐만 아니라, 의료 접근성이 강화되어 보다 포괄적이고 환자 중심적인 의료체계 구축에 이바지할 수 있을 것으로 기대된다

영문 초록

This study explores the experiences of overseas-residing Korean elderly individuals in utilizing healthcare services in Korea and analyzes the gap between their expectations and the realities they encounter. Many Korean elderly individuals prefer Korean healthcare services even while living abroad. Therefore, this study examines the key experiences and challenges they face when seeking medical care in Korea. A qualitative research approach was employed to gain an in-depth understanding of these experiences. In-depth interviews were conducted with 12 Korean elderly individuals aged 65 and older residing in the United States, collecting firsthand accounts of their healthcare service usage, the discrepancies between expectations and reality, and areas for improvement. The interview data were analyzed using thematic analysis to identify key patterns and themes. To ensure the reliability and validity of the study, participants' statements were repeatedly verified, and the results were cross-checked with similar studies for consistency. The findings revealed that overseas-residing Korean elderly individuals had high expectations regarding the speed and quality of healthcare services in Korea. They particularly felt psychologically reassured by their ability to communicate smoothly with Korean healthcare providers compared to their experiences with foreign healthcare systems. However, several challenges emerged. First, the lack of health insurance coverage for overseas residents resulted in a significant financial burden when accessing healthcare services. Second, the fast-paced nature of Korea’s medical system limited consultation time with healthcare providers, leading to concerns about insufficient communication. Third, unlike the patient-centered approach commonly experienced in the U.S. healthcare system, Korea’s healthcare services remained predominantly physician-centered, which felt unfamiliar to many overseas-residing elderly individuals. Based on these findings, implementing the proposed improvements could enhance the experiences of overseas-residing Korean elderly individuals in accessing healthcare services in Korea, improve medical accessibility, and contribute to the development of a more inclusive and patient-centered healthcare system.
