국문 초록

본 연구는 한국과 일본의 ALS환자와 그 가족들에 대한 지원체계를 Gilbert와 Terrell의 산출분석틀을 이용하여 분석하였다. 이를 통해 양국 지원체계의 차이점을 규명하고, 개선방안을 모색하는 것을 목적으로 하였다. 분석결과를 바탕으로 개선방안을 제시하면 다음과 같다. 첫째, ALS환자들에 대한 지원을 위해 부양의무자 기준을 축소해야 할 필요가 있다. 중증의 난치성 질환인 ALS환자와 그 가족에 대한 지원은 소득이나 재산이 아니라 중증 및 필요의 정도에 따른 지원이어야 한다. 이를 위해서는 우선적으로 부양의무자 기준을 축소하여 보다 실질적인 지원이 이루어질 수 있도록 하여야 할 것이다. 둘째, ALS환자에 대한 정확한 실태를 파악하고 이들에 대한 구체적이고 다양한 지원체계와 서비스개발에 대한 모색이 필요하다. 현재 제공되는 의료중심의 지원이 아닌 삶의 질을 높일 수 있도록 의료와 복지가 연계되어 제공되어야 한다. 셋째, 현재 산발적으로 이루어지고 있는 지원체계의 정비가 필요하다. ALS환자에 대한 정부주도의 연구나 임상실험을 활성화할 뿐만 아니라 현재의 희귀난치성 질환 관련 정보와 데이터의 구축을 위해 운영 중인 헬프라인의 기능도 보완하는 등 전달체계의 재정비가 필요하다. 넷째, 지원의 폭은 확대하고 있으나 예산은 감축하고 있는 것으로 나타났으므로, 사회적 합의의 도출을 통해 재정적 지원을 확보하거나, 특별후원금 등을 마련·적립하여 재정적 안정성을 확보할 필요가 있는 것으로 사료된다. 다섯째, 정책은 당사자들의 목소리가 바탕이 되어 개선되어야 한다. 따라서 당사자의 의견과 ALS협회와 같은 당사자 단체들의 목소리를 통해 정책개선이 되어야 하며, 이를 위해 당사자 단체의 육성과 지원이 필요하다.

영문 초록

The purposes of this study were to identify the differences of both countries support systems by analyzing the support systems on ALS patients and their families and to seek improvement measures based on them. The improvement measures based on the result of the analysis are; First, it needs to reduce the standard of people who are responsible for dependents for the support of ALS patients. The support of ALS patients which are severe incurable disease and their families should be focused on the degree of severity and necessity rather than their income or property. For this, it should make sure that the actual support is made by reducing the standard of people who are responsible for dependents. Second, it needs to figure out exact current situation on ALS patients and seek for the development of detailed and various support systems and services for them. The combined medical service and welfare should be supplied to increase the quality of life rather than medical centered support that are supplied currently. Third, the support system needs to be arranged as they are happening sporadically. It needs to rearrange the delivery system in activating government oriented studies and clinical tests on ALS patients and supplementing the function of help line which is operated to build the information and data related to rare incurable diseases. Fourth, as it showed that the range of support was expended but the budget was reduced, it is thought that it needs to be secure the financial stability by inducing the social agreement or prepare special support funds. Fifth, the policy should be improved based on voices of parties concerned. Thus, the policy should be improved according to their opinions and voices of groups concerned such as ASL Association and for that, it needs to foster and support of groups concerned.
