양측 인공와우이식이 청각장애 아동의 말지각에 미치는 영향

국문 초록

본 연구에서는 조용한 조건과 소음 조건에서의 일측 인공와우 아동, 양측 인공와우 아동, 정상청력 아동 간의 말지각 차이를 살펴보고, 소음 조건에서의 양측 인공와우 아동의 말지각 점수와관련 변수와의 상관관계를 살펴보고자 하였다. 생활연령이 7~13세인 일측 인공와우 아동(n=15),양측 인공와우 아동(n=15), 정상청력 아동(n=15)에게 조용한 조건과 소음 조건에서 단음절단어검사를 실시하였다. 연구결과, 모든 집단에서 조용한 조건에서의 음소점수가 소음 조건보다 유의하게 높았으며, 소음 조건에서의 음소점수는 정상청력 아동, 양측 인공와우 아동, 일측 인공와우아동 순서로 유의하게 높았다. 소음 조건에서의 양측 인공와우 아동의 말지각 점수, 첫 번째와두 번째 인공와우이식 연령, 양측 인공와우이식 간격 간에 유의한 부적 상관을 보였다. 양측 인공와우이식은 조용한 조건과 소음 조건에서 청각장애 아동의 말지각 향상에 도움을 주며, 양측 인공와우이식 연령이 낮고 양측 인공와우이식 간격이 짧을수록 소음 조건에서 양측 인공와우 아동의 말지각이 우수하였다. 그러므로 고도 이상의 청각장애 아동에게는 조기에 양측 인공와우이식을 권유하여 더 나은 청각적 수행력을 지닐 수 있도록 해야 할 것이다.

영문 초록

The purposes of this study were to investigate speech perception in quiet and noise condition for children with bilateral cochlear implants (BiCIs) in comparison with children with unilateral cochlear implants (UniCIs) and children with normal hearing (NH), and to investigate the relationship between speech perception scores in noise and related factors for children with BiCIs. Fifteen children with BiCIs, 15 children with UniCIs, and 15 children with NH were included in this study, and their ages ranged from 7 to 13 years. Monosyllabic word tests were used to examine their speech perception with and without noise. Children with BiCIs performed significantly better than children with UniCIs on monosyllabic word tests with and without noise, although children with BiCIs scored lower than children with NH on the tests. Speech perception scores in noise were significant correlated negatively with age at the first CI, age at the second CI and inter-implant interval in children with BiCIs. Children with BiCIs obtained better speech perception in quiet and noise condition than children with UniCIs. An age at the first CI, age at the second CI and inter-implant interval should be considered for counseling children with CIs and their parents about the effectiveness of the second CI
