중증 외상성 뇌손상 환자의 ICF 기반 방문 작업치료 효과: 사례연구

국문 초록

연구목적 : 본 연구의 목적은 ICF 기반 방문 작업치료 중재가 중증 외상성 뇌손상 환자의 일상생활활동 수행 능력, 낙상 빈도 감소, 그리고 사회적 참여 증가에 미치는 효과를 분석하는 것이다. 연구방법 : 본 연구는 중증 외상성 뇌손상 환자 1명을 대상으로 ICF 기반 평가를 통해 중재 전후 변화를 분석한 단일 사례 연구이다. 8개월 동안 월 2회씩 총 16회의 방문 작업치료를 실시하였으며, 일상생활활동 수행 능력, 낙상 빈도, 사회적 참여 변화를 측정하고 정량적⋅정성적 방법을 활용하여 효과를 검증하였다. 연구결과 : 방문 작업치료를 통해 일상생활활동 수행 능력이 41.67% 향상되었으며, 낙상 빈도는 월 평균 3회에서 0.5회로 감소하였다. 또한, 사회적 참여가 증가하여 활동보조인의 지원을 받아 외출 및 지역 모임에 다시 참여하였다. 결론 : 본 연구는 방문 작업치료가 중증 외상성 뇌손상 환자의 일상생활활동 수행 능력 향상, 낙상 빈도 감소, 그리고 사회적 참여 증가에 효과적임을 확인하였다. 본 연구는 단일 사례 연구로 일반화에는 한계가 있으나, 방문 작업치료의 실천적 가치를 확인하였으며, 향후 다양한 사례를 포함한 대규모 연구와 장기적 효과 검증이 필요함을 시사한다.

영문 초록

Objective : This study aimed to investigate the therapeutic efficacy of home-based occupational therapy interventions grounded in the International Classification of Functioning, Disability and Health (ICF) framework on functional independence in activities of daily living (ADLs), fall frequency reduction, and enhancement of social participation in an individual with severe traumatic brain injury (TBI). Methods : A single-subject case study design was employed involving a patient diagnosed with severe TBI. Sixteen sessions of ICF-oriented home occupational therapy were administered biweekly over an eight-month period. Pre- and post-intervention assessments were conducted using both quantitative and qualitative methodologies to examine changes in ADL performance, fall incidence, and levels of social engagement. Results : Post-intervention analysis revealed a 41.67% improvement in ADL performance. The monthly average fall rate significantly decreased from 3.0 to 0.5. Additionally, the participant demonstrated meaningful reintegration into community life, participating in local gatherings and outdoor activities with the assistance of a personal care aide. Conclusion : Findings from this case study underscore the potential clinical utility of ICF-based home occupational therapy in promoting functional recovery, mitigating fall risks, and fostering social reintegration in individuals with severe TBI. Although the single-case methodology limits generalizability, the outcomes suggest promising directions for future large-scale, longitudinal investigations.
