국문 초록

원발성 진행성 실어증(PPA)은 의미형, 비유창형, 로고페닉형의 세 아형에 따라 언어 손상의 양상이 상이하게 나타나므로, 정확한 감별 진단과 중재 전략 수립을 위해 아형별 언어 특성을 반영한평가도구의 개발이 요구된다. 본 연구는 기존 PPA 평가도구를 분석하고, 전문가를 대상으로 한 내용타당도 검증을 통해 임상 적용 가능한 하위 평가과제를 예비적으로 도출하는 것을 목적으로 하였다. 선행 평가도구인 PALS, PASS, SydBat를 중심으로 구성 요소와 평가 항목을 비교, 분석하여의미처리 기반 언어 과제의 필요성을 도출하였고, 이를 바탕으로 예비 문항을 구성하였다. 이후 언어병리 전문가 10인을 대상으로 문항별 적절성, 명확성, 대표성에 대한 평정을 실시하였다. 그 결과, 의미처리-어휘 접근 능력 등을 평가할 수 있는 저빈도 이름대기, 단어 정의하기, 단어연상과제가 최종 선정되었다. 각 과제는 세 아형 간 언어적 특성 차이를 반영한 진단적 변별성을 확보하였으며, 실질적인 활용 가능성이 검증되었다. 향후 본조사를 통해 임상자료 기반의 진단변별도 확보에 대한후속연구를 수행한다면, PPA 아형 감별을 위한 실용적 평가도구로 유용히 활용될 수 있을 것이다.

영문 초록

Primary progressive aphasia (PPA) manifests distinct patterns of language impairment depending on its three subtypes—semantic, nonfluent/agrammatic, and logopenic. Therefore, the development of subtype-specific assessment tools is essential for accurate differential diagnosis and effective intervention planning. This study aimed to analyze existing PPA assessment tools and preliminarily identify clinically applicable subtests through content validity evaluation by experts. Major components and assessment items from established tools such as PALS, PASS, and SydBat were compared and analyzed, highlighting the need for semantic processing-based language tasks. Based on this analysis, preliminary items were developed and evaluated by ten speech-language pathology experts for their appropriateness, clarity, and representativeness. As a result, low-frequency confrontation naming, word definition, and word association tasks—targeting semantic processing and lexical access—were selected. These tasks demonstrated diagnostic discriminability reflective of linguistic differences among PPA subtypes and were rated by experts as clinically feasible. Future studies involving clinical data will be necessary to establish diagnostic validity, which may enhance the utility of these tools for practical subtype differentiation in PPA.
