치매노인의 주간보호시설 이용 전후에 따른 주부양자의 부양경험 비교연구

국문 초록

질적연구방법인 심층면접을 통해 치매노인의 주간보호시설 이용 전과 비교하여 이용 후에 사회적 지지의 원천, 부양의식, 가족관계에 있어서 주부양자의 부양경험에는 어떠한 변화가 있으며, 이러한 변화를 가져온 구체적인 요인이 무엇인지를 파악해보고자 하는 것이 본 연구목적이다. 이를 위해 주간보호시설을 이용해오고 있는 65세 이상 치매노인의 주부양자 10명을 심층 면접하였다. 분석결과, 최종적으로 42개의 하위개념, 20개의 하위범주와 3개의 범주가 도출되었으며 주요 연구결과는 다음과 같다. 첫째, 주부양자들은 노인부양의 원조를 가족들에게만 매달려오던 것에서 벗어나, 치매노인의 주간보호시설 이용을 통해 가족외부(사회복지사, 자조모임, 각종 서비스)로 눈을 돌리게 됨으로써 부족했던 사회적 지지를 보충할 수 있게 되는 긍정적인 경험을 한 것으로 나타났다. 둘째, 주부양자들은 그동안 가급적 노인을 집에서 가족이 돌보는 것이 최선이라는 부양의식에서 벗어나 주간보호시설과 같은 가족외적 자원을 활용하는 것이 방법이 될 수 있음을 깨닫게 된다. 셋째, 주부양자들은 치매노인의 주간보호시설 이용을 통해, 치매노인과의 관계나 부부(자녀)관계가 좋아지게 되는 긍정적인 경험을 한 것으로 나타났다. 그러나 이러한 긍정적인 경험과는 달리 몇 가지 부정적인 경험도 나타났으며 이를 개선하기 위한 구체적인 제언을 제시하였다. 본 연구 결과는 주간보호시설을 이용하는 치매노인 주부양자의 부양경험을 이해하는데 보다 깊이 있는 자료를 제공해 줄 뿐만 아니라 구체적인 서비스 및 프로그램 개발을 위한 기초자료로 활용이 가능할 것으로 기대한다.

영문 초록

An in-depth interview research and qualitative methodology was used to find changes in the experiences of primary caregivers by the use of daycare centers for the senile elderly. Using 10 subjects, the following points were evaluated in detail. First, I looked at what kind of changes in the caregiving burden these subjects experienced after working at a senile-elderly daycare center, and what the specific, underlying reasons for the changes in their caregiving experience were. As a result of this analysis, 42 sub-concepts, 20 sub-categories and three categories were extrapolated. The three categories were 'source of social support,' 'consciousness of caregiving' and 'family relationship.'Generally, the primary caregivers, after using an adult daycare center, manifested a positive change, as they were able to offload some of the burdensome workload of caregiving. An important factor in this change is the use of public resources such as adult daycare facilities that operate outside of the circle of each respective patient's family. These facilities provided the primary caregiver with rest, lessened the heavy burden of caregiving and provided a way to lessen family feuding over the issue of caring for elders by providing inexpensive service. The following specific factors were the main mediating factors for using such a facility: meal service, vehicle service, caregiver education, caregiver consulting and other services, social support from social workers and dementia family group, and low financial burden.
