A case of bilateral subcapsular renal hematomas in 30-year-old women

A subcapsular renal hematoma is uncommon in clinical practice. Here, we present a case of 30-year-old woman who arrived at the emergency room with lower abdominal pain for two weeks, which radiated to the lower back. She had no underlying diseases. The abdomino-pelvis computed tomography showed a subcapsular hematoma in both kidney, but there was no definite active bleeding or mass-like lesion. The patient was admitted for a work-up to find the etiology and decide the appropriate treatment. The results of coagulation tests, autoimmune antibody tests, and renal arteriography were normal. The origin of the bilateral renal hematomas was considered as idiopathic or delayed traumatic. She was treated with conservative management. Three months later, the bilateral subcapsular hematomas began to decrease without invasive procedures. She continued to be followed in an outpatient setting without any serious complications. Conservative management may be appropriate for hematomas that are hemodynamically stable and bilateral.
