후방유수정체안내렌즈 삽입술과 전층 난시교정각막절개술의 동시 수술의 단기간 임상성적

국문 초록

목적: 중등도 이상의 난시 및 근시 환자를 대상으로 후방유수정체안내렌즈(implantable collamer lens, ICL) 삽입술과 전층 난시교정 각막절개술을 동시에 시행하여 임상성적을 평가하고자 하였다. 대상과 방법: 16명 32안을 대상으로 ICL 삽입술과 전층 난시교정각막절개술을 동시에 받은 환자를 대상으로 연구를 시행하였다. 술 전과 술 후 1주, 1개월, 3개월에 나안시력, 굴절력, 효율성, 안전성, 예측성 등을 비교하였다. 결과: 술 전과 비교하여 술 후 3개월째 교정된 난시의 양은 74.83 ± 13.8%였고, 구면렌즈대응치가 ±0.5D 이내인 경우는 87.5%였으 며, 100%에서 ±1.0D 이내로 교정되었다. 나안시력이 20/25 이상인 비율이 100%, 20/20 이상인 비율은 81.25%였으며, 재수술은 저 교정 때문에 1안(3.1%)에서 시행되었고, 합병증은 관찰되지 않았다. 결론: ICL 삽입술과 전층 난시교정각막절개술의 동시 수술은 중등도 이상의 난시 및 근시를 효과적으로 교정할 수 있는 안전한 수술로 생각된다.

영문 초록

Purpose: To evaluate the clinical outcomes of implantable collamer lens (ICL) implantation with simultaneous full thickness as-tigmatic keratotomy (FTAK) for the correction of moderate to high myopic astigmatism. Methods: Thirty-two eyes of 16 patients who had an ICL implantation with simultaneous FTAK were studied. Follow-up visits were at 1 week, 1 month, and 3 months. The outcome measures included the uncorrected distance visual acuity (UDVA), re-fractive error, efficacy, safety, and predictability. Results: After the surgery, astigmatism was reduced by 74.83 ± 13.8%. The proportion of eyes with a spherical equivalent of 0.5 D or less was 87.5%, and all eyes had a spherical equivalent of 1.0 D or less at 3 months after the surgery. The proportion of eyes with a UDVA of 20/25 or better was 100%, and 20/20 or better was 81.25%. Reoperation was needed in one case (3.1%) because of undercorrection of the astigmatism, and no complications were observed. Conclusions: This study showed that ICL implantation with simultaneous FTAK is effective and safe for the correction of moder-ate to high myopic astigmatism.
