국문 초록

목적: 아메드밸브삽입술 후 맥락막박리 및 전방소실의 위험인자에 대해 알아보고자 하였다. 대상과 방법: 아메드밸브삽입술을 시행받은 녹내장 환자 중 1년 이상 경과한 94명 94안을 대상으로 후향적 의무기록 분석을 시행하였다. 나이, 성별, 안압, 녹내장 종류, 안축장, 중심각막두께, 수정체 상태 및 전신 질환을 비교 분석하였다. 결과: 22안에서 수술 후 맥락막박리를 보였고, 8안에서 수술 후 전방소실이 나타났다. 맥락막박리를 보인 군에서는 맥락막박리를 보이지 않은 군에 비해 고령, 높은 술 전 안압, 짧은 안축장, 당뇨, 고혈압, 인공수정체안, 낮은 MD값을 보였다. 다변량 분석을 시행한결과 나이(p=0.005), 술 전 안압(p=0.024), 신생혈관녹내장(p<0.001), 당뇨(p=0.025), 고혈압(p=0.018)이 아메드밸브삽입술 후 맥락막박리의 발생과 유의한 연관성을 보였다. 전방소실을 보인 군에서는 전방소실을 보이지 않은 군에 비해 고령, 높은 술 전 안압, 짧은 안축장, 당뇨, 고혈압, 인공수정체안, 낮은 mean deviation 값을 보였다. 다변량 분석을 시행한 결과 나이(p<0.001), 술 전 안압(p<0.001), 안축장(p=0.022), 신생혈관녹내장(p<0.001), 고혈압(p=0.036)이 아메드밸브삽입술 후 전방소실의 발생과 유의한 연관성을 보였다. 결론: 아메드밸브삽입술 후 발생하는 맥락막박리의 위험인자는 고령, 높은 술 전 안압, 신생혈관녹내장, 당뇨, 고혈압이었고, 전방소실의 위험인자는 고령, 높은 술 전 안압, 짧은 안축장, 신생혈관녹내장, 고혈압이었다.

영문 초록

Purpose: To investigate risk factors associated with choroidal detachment and flat anterior chamber following Ahmed valve implantation. Methods: This retrospective study reviewed of 94 glaucomatous eyes from 94 patients who underwent Ahmed valve implantation with a follow-up period of at least 1 year. Preoperative age, sex, intraocular pressure (IOP), glaucoma type, axial length, central corneal thickness, lens status, and systemic diseases were evaluated. Results: Following Ahmed valve implantation, choroidal detachment occurred in 22 eyes, and a flat anterior chamber developed in eight eyes. Eyes with choroidal detachment were associated with older age, higher preoperative IOP, shorter axial length, diabetes, hypertension, pseudophakia, and lower mean deviation (MD) on visual field testing. Multivariate analysis identified significant associations of postoperative choroidal detachment with age (p = 0.005), preoperative IOP (p = 0.024), neovascular glaucoma (p< 0.001), diabetes (p = 0.025), and hypertension (p = 0.018). Similarly, eyes with a flat anterior chamber were associated with older age, higher preoperative IOP, shorter axial length, diabetes, hypertension, pseudophakia, and lower MD. Multivariate analysis revealed significant associations of a postoperative flat anterior chamber with age (p < 0.001), preoperative IOP (p < 0.001), axial length (p = 0.022), neovascular glaucoma (p < 0.001), and hypertension (p = 0.036). Conclusions: The risk of choroidal detachment after Ahmed valve implantation is significantly associated with older age, high preoperative IOP, neovascular glaucoma, diabetes, and hypertension. Similarly, the risk of a flat anterior chamber is associated with older age, high preoperative IOP, short axial length, neovascular glaucoma, and hypertension.
