제도분석틀 수정모형(IAD)을 적용한 의과대학 증원 갈등 사례분석

국문 초록

본 연구는 윤석열 정부가 추진한 의과대학 정원 확대 정책을 둘러싼 갈등 양상을 제도적 관점에서 분석하기 위해 Ostrom의 제도분석틀(IAD)과 사회생태시스템(SES) 모형을 결합한 통합 분석틀을 적용하였다. 정책의 외생적 조건(물리적 조건, 공동체 속성, 사용 규칙)과 사회적 맥락(정치･경제･생태 환경), 그리고 행위자들의 선호와 인식, 전략적 상호작용 과정을 분석함으로써, 단순한 정원 확대를 넘어서는 제도적 충돌과 신뢰의 문제를 구조적으로 설명하였다. 분석 결과, 물리적 조건에서는 공공의료 인프라 취약과 지역 간 의료자원 불균형이 갈등의 구조적 전제가 되었고, 공동체 속성에서는 전문직 자율성과 정체성에 대한 의료계의 규범이 집단행동 결속을 강화했으며, 사용 규칙에서는 고등교육법상 정원 통보 절차와 수련･국시･수가 체계 등 하위 규칙 간 정합성 결여가 제도 충돌을 심화시켰다. SES 관점에서 초고령화･필수의료 공백･감염병 대비와 같은 사회･생태적 압력이 정책의 시급성을 높였으나, 거버넌스 시스템의 숙의･조정 기능 부재로 정책 추진은 ‘일방 통보–집단행동–대립고착’의 경로를 밟았다. 자원 단위(정원 수)의 경우, 정부 초기안과 최종 확정의 규모･배분 변화가 행위자 기대･반발을 재조정하는 매개로 작용했다. 종합하면, 본 연구에서 수정 IAD 모형은 ‘규칙 정합성 결여 → 행위상황의 정면충돌･조정 실패 → 갈등 장기화’와 ‘거버넌스 협의 부재 → 신뢰 저하 → 수용성 약화’라는 연결메커니즘을 제시한다. 정책적 시사점으로는 정원 확대의 전제조건으로 교육･수련･공공병원 역량에 대한 수용 여력 진단과 공개, 대표성･정보대칭･의제 범위･구속력 기준을 갖춘 사회적 협의체 설계, 고등교육–수련인증–수가–국시–지역의사제 간 규칙 정합성 체계적 점검, 증원 규모･배분･이행 일정･보완 재정 등 주요 조정 가능 변수의 단계적 조정을 제안한다. 학술적 기여는 IAD에 행위자 선호･인식과 거버넌스–규칙 정합성 점검 매트릭스를 접목하여 의료정책 갈등의 제도적 원인과 상호작용 경로를 분석모형으로 구체화했다는 데 있다.

영문 초록

This study examines the conflict surrounding the Yoon Suk-yeol administration’s expansion of medical school admissions through an integrated framework that combines Ostrom’s Institutional Analysis and Development (IAD) and the Social-Ecological System (SES) model. By analyzing exogenous conditions—physical conditions, community attributes, and rules-in-use —together with the broader political, economic, and ecological context and with actors’ preferences, perceptions, and strategic interactions, the study explains institutional collisions and trust deficits that extend beyond a simple seat increase. The findings indicate that weak public-healthcare infrastructure and spatial maldistribution of medical resources formed structural preconditions for conflict (physical conditions); professional norms surrounding autonomy and identity strengthened collective-action cohesion in the medical community (community attributes); and misalignment among the Higher Education Act–based quota notification procedure and lower-level regimes for residency training, national licensing, and the fee schedule intensified rule collisions (rules-in-use). From an SES perspective, aging, gaps in essential services, and pandemic preparedness pressures heightened policy urgency, while the absence of deliberative and coordinating capacities in the governance system set the process on a path of unilateral notification, collective action, and entrenched confrontation. At the level of the resource unit (admissions quota), shifts in scale and allocation between the initial proposal and the final decision mediated actors’ expectations and backlash. Synthesizing these results, the modified IAD proposed here specifies two linkage mechanisms—rule-coherence deficits leading to confrontation and adjustment failure with prolonged conflict, and lack of negotiated governance leading to erosion of trust and weakened acceptance—and offers policy implications on capacity assessment and disclosure, the design of a representative and information-symmetric social deliberation body, systematic rule-coherence checks across adjacent regimes, and phased adjustment of tunable variables. The scholarly contribution lies in operationalizing IAD with actors’ preferences and perceptions and a governance–rule-coherence audit matrix to trace institutional causes and interaction pathways in healthcare policy conflicts.
