국문 초록

Patients with cleft palate often present with a narrow and retruded maxilla, transposed teeth, congenital tooth loss, upper lip retrusion, and a prognathic mandible. Previous studies indicate that tooth transposition occurs more frequently in the maxilla than in the mandible, with maxillary canines and first premolars being the most commonly affected, followed by canines and lateral incisors. In adult patients with cleft palate, orthodontic treatment combined with orthognathic surgery is often necessary to address a retruded maxilla or a relatively prognathic mandible. When transposed teeth are present, the surgical plan must be carefully tailored to accommodate their unique positioning. This case report discusses the diagnosis and treatment planning of a 20-year-old female patient who visited our clinic with the chief complaint of a prognathic mandible and anterior crowding. Diagnostic examination revealed a cleft palate, a transposed maxillary left canine, and a maxillary left central incisor with severe root resorption. Through comprehensive evaluation, this report explores key diagnostic considerations and treatment strategies, including the management of transposed teeth, maxillary expansion for patients with cleft palate, the prognosis of affected teeth, and the long-term stability of orthodontic treatment.
