국문 초록

The treatment of adult cleft lip and palate patients with severe facial asymmetry and skeletal Class III malocclusion requires orthognathic surgery. This report discusses the case of a 19-year-old man with a unilateral cleft lip and palate, skeletal Class III malocclusion, facial asymmetry, and a significant arch width discrepancy. To address the transverse discrepancy, maxillary arch expansion was performed using a hyrax expander, followed by presurgical orthodontic treatment. The dental arch was decompensated through the use of a resin bite, mini-implants, and criss-cross elastics. After completing presurgical orthodontic treatment, orthognathic surgery was conducted, involving a Le Fort I osteotomy for the maxilla and a combination surgical approach for the mandible. Following postsurgical orthodontic treatment, skeletal and dental issues were successfully corrected, resulting in improved facial aesthetics. (Clin J Korean Assoc Orthod 2025;15(2):158-169)
