국문 초록

Cleft lip and palate often result in malocclusion and maxillary hypoplasia, even after comprehensive treatment. While various surgical and orthodontic approaches address these discrepancies, many have limitations such as velopharyngeal incompetence or inability to alter maxillary arch length. Maxillary anterior segmental distraction osteogenesis (MASDO) provides a solution by advancing the anterior maxillary segment, creating alveolar bone space, and maintaining velopharyngeal function. Therefore, the purpose of this presentation is to demonstrate the successful application of MASDO in a patient with cleft palate and severe maxillary hypoplasia. A 17-year-old male visited the clinic with a chief complaint of anterior crossbite and severe crowding. He showed a skeletal Class III relationship with severe maxillary retrusion and collapsed maxillary arch. He had a history of cleft palate and had undergone palatoplasty when he was 11 months old. Treatment involved miniscrew-assisted maxillary expansion, followed by MASDO. Then extractions of upper and lower teeth were performed for decrowding. As a result, the anterior maxillary segment was advanced, enabling correction of the anterior crossbite and crowding. A functional occlusion with improved facial profile and smile esthetics was obtained. (Clin J Korean Assoc Orthod 2025;15(2):143-157)
