Introduction: The purpose of this study is to analyze the pattern and frequency of unilateral impaction of the second molar in subjects with facial asymmetries according to different anteroposterior skeletal patterns. Additionally, it aims to compare the lack of eruption space between the impacted side and the contralateral side with normal eruption. Methods: A total of 64 patients aged 12 years or older with facial asymmetry and unilateral impaction of the second molar were classified into three groups based on their anteroposterior skeletal pattern: Class I, Class II, and Class III. The frequency and percentage distribution of impaction were analyzed for each group. Furthermore, each group was further subdivided based on the location of the impacted tooth, and the posterior molar space was compared between the impacted and non-impacted sides. Results: The highest incidence of impacted teeth was observed in the Class II group. In the Class I group, impaction occurred equally in the DL (lower impaction in the deviated side) and NL (lower impaction in the non-deviated side) subgroups. In the Class II group, the DL subgroup had the highest occurrence of unilateral impaction. The posterior space of the mandibular first molar was significantly smaller on the impacted side in the Class I DL, Class II DL, and Class II NL subgroups. Conclusions: In patients with facial asymmetry, unilateral second molar impaction occurs most frequently in Class II cases and tends to result from space deficiency, primarily in the mandibular deviated side.
