설질과 설태 색상의 국가참조표준 개발 과정 및 제언

국문 초록

본 연구는 한의학에서 진단 방법으로 활용되어 온 설진 중 설질과 설태의 색상에 대한 성별 및 연령별 국가참조표준을 개발한 과정과 결과를 기술했다. 선행 연구에서 수집한 설 영상 측정 표준 운영 절차를 기반으로 데이터 생산 절차서 및 참조표준 생산/평가 절차서를 개발했다. 이 과정을 통해 측정 소급성 및 불확도를 기술했다. 절차서에 따라 선행 연구에서 구축한 혀 영상 데이터셋으로, 설질과 설태 영역은 다시 설첨, 설변, 설중, 설근으로 세부 구분하고, 세부 영역에 대한 참조표준을 개발했다. 선행 연구에서 혀 영상 획득 장치 및 시스템으로 수집한 2,158건의 한국인 정상인 혀 영상 데이터에서 967건을 추출했다. 동일한 혀를 5회 이상 반복 측정하여 세부 영역별 측정의 불확도를, 색상 보정 과정에서 발생하는 불확도를 산출 및 합성해 측정불확도를 도출했다. 성별에 따른 6개 연령군(20-29, 30-39, 40-49, 50-59, 60-69, 70-79세)의 12개 군에 대한 참조표준과 확장 불확도(k=2)를 도출하였다. 70대 연령군은 남녀 모두 수가 가장 적어 불확도가 크게 나타나, 향후 추가적인 데이터 수집의 필요성을 보여줬다. 설태의 CIE L*, b*값은 설질보다 높게 나타났는데, 이는 보통 반투명한 백색의 설태가 설질보다 밝기가 밝고 덜 붉은 특성이 반영된 것이다. 설질 설첨 영역의 CIE a*값이 가장 크게 나타났는데, 이는 혀의 해부생리학적 특성에 의한 것으로 보인다. 설질과 설태의 CIE b*값은 여성이 남성보다 높게 나타나며, 여성 혀의 붉기가 더 연하게 관찰됨을 나타냈다. 본 연구는 한국인 정상인의 설질 및 설태 색상의 국가참조표준을 개발함으로써 정상 혀의 기준을 세우고 설진 측정의 신뢰성 제고를 도모하고자 했다. 본 국가참조표준은 다양한 설 영상 측정 시스템에서 정상 참조치로 활용 가능하고, 교육이나 임상 연구, 나아가 모바일 건강 관리 분야에도 활용될 것으로 기대한다. 향후 고령층 대상자 데이터를 추가 수집하고, 색상 외 설진 지표에 대한 참조표준 개발이 이루어져야 할 것으로 보인다.

영문 초록

This study describes the development process and results of national reference standards for the color of tongue body and tongue coating by sex and age, based on tongue diagnosis, a traditional diagnostic method in Korean medicine. Standard operating procedures for tongue image measurement established in previous research were used to develop protocols for data production and reference standard evaluation, ensuring traceability and measurement uncertainty. Using a dataset of 2,158 tongue images from healthy Koreans, 967 images were selected for analysis. The tongue body and coating were further subdivided into tip, edge, center, and root regions, and reference standards were established for each subregion. The measurement uncertainty was derived by calculating and combining the uncertainties fromrepeated measurements (five or more times) of the same tongue for each specific area and the uncertainty arising from the color calibration process. Reference standards and expanded uncertainties (k=2) were established for 12 groups, consisting of six age groups (20–29, 30–39, 40–49, 50–59, 60–69, 70–79 years) by sex. The smallest sample size and highest uncertainty were found in the 70–79 age group, indicating the need for additional data collection in this demographic. The CIE L* and b* values for tongue coating were higher than those for the tongue body, reflecting the brighter and less reddish nature of the typically translucent white coating. The CIE a* value was highest in the tip region of the tongue body, likely due to anatomical and physiological characteristics. The CIE b* values for both tongue body and coating were higher in women than in men, indicating that women’s tongues tend to appear less red. By developing national reference standards for tongue body and coating color in healthy Koreans, this study establishes criteria for normal tongues and aims to enhance the reliability of tongue diagnosis measurements. These standards can serve as normal reference values for various tongue imaging systems and are expected to be useful in education, clinical research, and mobile health management. Further data collection in older adults and the development of reference standards for other tongue diagnostic indices beyond color are suggested for future research.
