작업치료 중재가 치매 환자의 정신건강에 미치는 영향: 체계적 고찰

국문 초록

목적: 본 연구는 작업치료 중재가 치매 환자의 정신건강에 미치는 영향을 체계적으로 고찰하여, 향후 치매관련 연구 및 실무에서 활용할 수 있는 기초자료를 마련하고자 한다. 연구방법: 2025년 1월까지 Embase, KCI, Pubmed, Web of science 데이터베이스의 문헌들 중 선정기준에 따라최종적으로 7편을 선정하여 일반적특성, 중재 유형 및 방법, 정신건강에 미치는 영향에 대해 분석하였다. 연구결과: 분석된 문헌들의 중재 유형은 회상치료 프로그램(작업 중심, 맞춤형) 2편, 지역사회 기반 작업치료(인지 및 행동 중재, 가정환경 수정), 라이프스타일 재설계 프로그램, 가정방문 인지 및 신체활동 작업치료 프로그램, 과제 지향적 활동 프로그램, 감각활동기반 인지재활 그룹 프로그램이 각 1편씩으로 나타났다. 분석 문헌들에서 정신건강 변수인 삶의 질, 우울, 자기효능감이 나타났으며, 삶의 질이6편으로 가장 많이 조사되었다. 모든 분석대상 문헌에서는 중재에 따라 유의미한 결과를 나타냈다. 결론: 본 연구는 체계적 문헌고찰을 통해 작업치료 중재가 치매 환자의 정신건강에 미치는 영향을 분석하였다. 연구 결과, 작업치료 중재는 우울 감소, 삶의 질 향상, 자아효능감 증진 등 긍정적인 영향을미치는 것으로 나타났다. 치매 환자의 정신건강 증진을 위해 전인적 접근이 필요하며, 향후 연구에서는 메타분석을 통한 효과 검증과 보다 정교한 중재 전략 개발이 요구된다.

영문 초록

Objective : This study systematically examined the effects of occupational therapy intervention on the mental health of dementia patients and aimed to provide basic data that can be used in future dementiarelated research and practice. Methods : Until January 2025, 7 articles were finally selected based on the selection criteria from Embase, KCI, Pubmed, and Web of Science databases, and their general characteristics, intervention types and methods, and effects on mental health were analyzed. Results : The types of interventions in the analyzed articles were 2 reminiscence therapy programs (occupationcentered, customized), 1 community-based occupational therapy (cognitive and behavioral intervention, home environment modification), 1 lifestyle redesign program, 1 home-visit cognitive and physical activity occupational therapy program, 1 task-oriented activity program, and 1 sensory activity-based cognitive rehabilitation group program. The analyzed articles showed mental health variables such as quality of life, depression, and self-efficacy, and quality of life was investigated the most with 6 articles. All analyzed articles showed significant results depending on the intervention. Conclusion : This study systematically reviewed the effects of occupational therapy interventions on the mental health of individuals with dementia. The results showed positive impacts, including reduced depression, improved quality of life, and enhanced self-efficacy. Personalized approaches and emotional support were key factors in maximizing effectiveness. Future research should conduct meta-analyses to validate outcomes and refine intervention strategies.
