불안을 호소하는 유방암 환자에 대한 수용전념치료 적용 사례

국문 초록

본 사례는 2024년 00 대학병원 의료사회복지팀에서 임상연수사회복지사로 근무하던 당시, 유방암 진단 이후 심리적 불안을 경험하는 47세 여성 환자를 대상으로 수용전념치료(Acceptance and Commitment Therapy, ACT)를 적용한 사례이다. 환자는 치료 과정에서 지속적인 불안, 우울, 침습적 사고 등 심리적 어려움을 호소하였으며, 이는 치료 집중의 저하로 이어졌다. 초기 상담에서는 디스트레스 평가, 한국판 우울증 선별 도구(PHQ-9), 불안장애 척도(GAD-7)를 통해 환자의 심리적 문제와 우울 및 불안 수준을 구체적으로 확인하였다. 이후 총 6회의 상담 과정에서 인지적 탈 융합, 마음 챙김, 수용, 가치 탐색 등 ACT 기법을 적용하여 환자가 자신의 상황을 기꺼이 경험하고 심리적 유연성을 발휘할 수 있도록 지원하였다. 개입 결과, 환자의 표정, 목소리, 활력 등에서 긍정적 변화가 관찰되었으며, 자기 보고식 표현을 통해 정서적 안정감 회복을 확인하였다. 본 사례는 ACT 기반 상담이 유방암 환자의 심리적 불안 완화 및 치료 적응 지원에 효과적일 수 있음을 시사하며, 의료사회복지 영역에서 ACT 활용의 필요성과 적용 가능성을 제시한다.

영문 초록

This case study reports an intervention conducted in 2024 at the Medical Social Work Team of 00 National University Hospital, where the author was a clinical social work trainee. The patient was a 47-year-old woman who experienced psychological distress following a breast cancer diagnosis. During treatment, she reported persistent anxiety, depression, and intrusive thoughts, which led to decreased treatment engagement. Initial assessments using the Distress Thermometer, the Korean version of the Patient Health Questionnaire-9 (PHQ-9), and the Generalized Anxiety Disorder-7 (GAD-7) scale identified the severity of her psychological problems. Six counseling sessions were conducted using Acceptance and Commitment Therapy (ACT) techniques, including cognitive defusion, mindfulness, acceptance, and values exploration, to support the patient in willingly experiencing her situation and enhancing psychological flexibility. Following the intervention, positive changes were observed in her facial expressions, voice, and vitality, and self-reported measures indicated a recovery of emotional stability. This case suggests that ACT-based counseling may be effective in alleviating psychological distress and supporting treatment adaptation in breast cancer patients, highlighting its potential applicability in medical social work.
