국문 초록

본 연구는 HIV/AIDS 감염인을 위한 호스피스·완화의료의 필요성과 현재 우리나라의 제도 및 현실적 상황을 조망하고, 외국의 사례를 분석함으로써 시사점을 찾기 위한 기초 고찰이다. 항레트로바이러스 치료의 발전으로 HIV는 생존 가능한 만성질환으로 변화하였으나, 감염인은 여전히 낙인, 고립, 다중 만성질환 등 복합적 고통을 경험하고 있다. 특히 말기 상태에 이르면 심리·사회·영적 고통이 심화되며, 이에 대한 총체적 대응으로서 호스피스·완화의료의 중요성이 강조된다. 2016년 「호스피스·완화의료 및 임종과정에 있는 환자의 연명의료결정에 관한 법률」 제정으로 HIV 감염인도 가정형·자문형 호스피스를 이용할 수 있는 법적 기반은 마련되었으나, 실제로는 감염관리 부담, 사회적 낙인, 인력과 인프라의 부재 등으로 인해 서비스 이용률이 극히 저조하다. 이에 본 연구는 HIV/AIDS 감염인을 위한 완화의료가 질병 전 경과에 걸쳐 조기 통합되어야 함을 강조하고, 가정형 및 자문형 호스피스 내에서 사회복지사의 역할, 지역사회 연계 중심의 호스피스·완화의료 네트워크의 필요성 등을 제시하였다.

영문 초록

This study is a foundational review aiming to explore the necessity of hospice and palliative care for people living with HIV/AIDS (PLWHA), assess the current institutional and practical landscape in South Korea, and identify implications through analysis of international models. Although antiretroviral therapy (ART) has transformed HIV into a manageable chronic condition, individuals living with HIV continue to experience complex suffering due to stigma, social isolation, and multiple comorbidities. In advanced stages of illness, psychological, social, and spiritual distress intensifies, underscoring the critical importance of comprehensive hospice and palliative care. Since the enactment of the 2016 “Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment for Patients at the End of Life,” PLWHA in Korea have been legally entitled to access home-based and consultative hospice services. However, in practice, service utilization remains extremely limited due to infection control concerns, persistent social stigma, and a lack of trained personnel and infrastructure. This study emphasizes the need to integrate palliative care throughout the disease trajectory for PLWHA and proposes the expansion of home-based and consultative hospice models. It also highlights the crucial role of social workers in these care settings and the importance of developing a community-based hospice and palliative care network tailored to the unique needs of people living with HIV/AIDS.
