국문 초록

목적: 상악 좌측 유견치의 만기잔존과 상악 좌측 견치 및 제1소구치의 이소맹출을 동반한 골격성 I급 환자에서 콘빔전산화단층촬영을 바탕으로 최적의 견인방향을 분석한 비발치 증례를 보고하고자 한다. 증례: 본 26세 남환은 골격성 I급으로서 상악 좌측 유견치의 만기 잔존, 상악 좌측 견치와 제1소구치의 이소맹출 및 하악 전치부 총생을 보였다. 상악에 Nance holding arch를 사용하여 견치를 전방으로 견인하였고, 제1소구치의 회전을 조절하여 치열궁 내 배열하였다. 하악의 경우 미니스크류를 고정원으로 하여 부분적 원심이동을 통해 하악 전치부 배열을 마무리하였다. 결론: 상악 좌측 견치 및 제1소구치의 이소맹출을 동반한 환자에서 적절한 고정원을 사용하고, 치아의 정확한 위치 관계를 파악한 후 최적의 견인 방향을 설정하여 성공적으로 치아를 배열하였다.

영문 초록

Purpose: We report a case of non-extraction in a patient with skeletal Class I malocclusion and retained maxillary left primary canine and ectopic eruption of the maxillary left canine and first premolar, in which the optimal direction of traction was analyzed using cone-beam computed tomography. Case Report: A 26-year-old male patient with skeletal Class I malocclusion presented with retention of the maxillary left primary canine, ectopic eruption of the maxillary left canine and first premolar, and crowding of the mandibular anterior teeth. The maxillary canine was pulled forward using a Nance-holding arch, and the rotation of the first premolar was adjusted to align it within the dental arch. Alignment of the mandibular anterior teeth was completed through segmental distalization using miniscrews for anchorage. Conclusion: The teeth were successfully aligned in this patient with ectopic eruption of the maxillary canine and first premolar using an appropriate anchorage device, determining the exact positional relationship of the teeth and establishing the optimal direction of traction.
