Minimally invasive immediate implantation in the anterior region affected by aggressive periodontitis: two case reports

This report describes two clinical cases of immediate implant placement in the anterior maxilla and mandible of patients diagnosed with aggressive periodontitis. Both cases had significant soft tissue defects, including alveolar bone loss, gingival swelling, and recession. Despite these compromised conditions, a minimally invasive, flapless surgical approach was used. Bone grafting using a freeze-dried bone allograft (FDBA) was performed without barrier membranes, even in the presence of periosteal defects. Secondary wound healing was allowed rather than pursuing primary closure. Favorable clinical outcomes were achieved in both cases. Hence, immediate implant placement, using a minimally invasive technique and limited grafting, is feasible even in anterior regions with extensive periodontal destruction without requiring flap elevation or membrane coverage.
