상악 좌측 측절치 결손을 동반한 골격성 II급 환자의 하악 전치 발치 치험례

국문 초록

목적: 상악 좌측 측절치 결손과 하악 우측 견치의 이소맹출을 동반한 골격성 II급 환자에서 하악 우측 측절치의 전략적 발치를 통해 상하악 좌측에서 견치 대체가 성공적으로 이루어진 증례를 보고하고자 한다. 증례: 본 16세 남환은 윗 치아 사이에 공간이 있고 아래 치아가 고르지 않다는 주소로 본원에 내원하였다. 상악 좌측 측절치 결손은 공간 폐쇄를 통한 견치 대체를 계획하였으며 하악 우측 측절치 발치를 통해 총생의 해소와 정중선 변위를 해결하고자 하였다. 미니스크류를 사용하여 상악 좌측 구치부의 전방이동을 시행하였고, 대체된 치아들의 미세심미와 교합관계 향상을 위해 치아모양 재형성, 치아 크기 조절, 적절한 본딩과 마무리를 시행하였다. 결론: 상하악 좌측 견치와 제1소구치가 각각 측절치와 견치를 성공적으로 대체하게 되었고 양측 I급 견치 및 구치관계와 적절한 수직, 수평피개를 형성하여 기능적인 교합을 달성하였다. 측절치를 대체한 상하악 좌측 견치의 미세 심미도 바람직하였다. 한편, 치료 기간 동안 하악의 바람직한 전방성장이 이루어져 볼록한 안모가 개선되었다.

영문 초록

Purpose: This case report aims to present a successful case of mandibular incisor extraction and strategic canine substitution in the left maxillary and mandibular regions in a patient with skeletal Class II malocclusion accompanied by a congenitally missing maxillary left lateral incisor and ectopic eruption of the mandibular right canine. Case Report: A 16-year-old boy presented to our clinic with a chief complaint of spacing between the upper teeth and crowding of the lower teeth. Canine substitution was planned to replace the missing maxillary left lateral incisor, and extraction of the mandibular right lateral incisor was performed to resolve crowding and midline deviation. Protraction of the maxillary left molar was performed using miniscrews, and meticulous reshaping of the substituted teeth, adjustment of the tooth size, appropriate bracket bonding, and finishing were performed to enhance the esthetics and occlusal relationship. Conclusion: Canine substitution on both the left maxillary and mandibular sides was successfully performed, and a Class I canine and molar relationship was achieved bilaterally. Functional occlusion was achieved by establishing ideal vertical and horizontal overlaps. The esthetic outcomes achieved by the maxillary canine, which replaced the missing lateral incisors, were satisfactory. Additionally, desirable forward growth of the mandible occurred during the treatment period, resulting in an improvement in the convex facial profile.
