장애인정책에서의 보건·복지 협력에 대한 신제도주의 분석

국문 초록

사회보장정책뿐 아니라 장애인정책에서도 보건·복지 간 협력은 정책의 다기화 및 수준 제고에 따라 당사자 욕구에 대한 책임성뿐만 아니라 정책의 효과성과 효율성 확보를 위한 중요한 정책 중의 하나로 볼 수 있다. 이에 본 연구에서는 장애인정책에서의 보건·복지 협력에 대해 신제도주의에서의 제도적 복합성과 제도적 논리라는 개념으로 분석하였다. 그 결과, 정책영역에서 보건·복지 정책의 미분화와 양 영역 간의 접점 부족 그리고 자립생활모델 및 사회적모델의 도입과 지역사회 통합돌봄의 영향력이, 거버넌스 영역에서는 보건·복지 협력에의 법률 개념의 비체계성과 양 영역 간의 배타성, 보건 중심의 CBR사업과 복지 중심의 통합사례관리 및 장애인전담민관협의체의 이원적 병존이, 시간 영역에서는 제도의 존속 및 발전에 기여하는 긍정적 피드백이 지배적인 점 등이 복합적으로 제도적 복잡성을 구성하고 있음을 알 수 있다. 보건과 복지 중 어느 것이 중심이 되어야 하는지에 대한 제도적 논리가 지금 현재 낮은 정도의 협력수준에서는 문제가 되지 않으나 앞으로 협력수준이 더 고도화되면 양 영역 모두 제도의 중심성을 차지하기 위해 조정 및 갈등의 양상이 일어날 것으로 보인다. 본 연구는 많은 연구가 이루어지지 않은 장애인정책에서의 보건·복지 협력이슈를 다루고 있다는 점, 현상을 통해 문제점 및 대책을 진단하기보다는 이론적 틀로 제도 변화를 둘러싼 다양한 측면을 처음 분석했다는데 그 의의가 있다. 그럼에도 불구하고 관련 정책을 둘러싼 이해관계자의 폭넓은 의견을 청취하거나 장애인 당사자의 문제의식에서 출발한 것이 아니라는 점에서 한계를 지니는데 이는 후속연구로 보완되기를 기대한다.

영문 초록

The importance of cooperation between public health and social welfare policy for disabled people is ever-increasing to meet responsiveness of the needs represented, and effectiveness and efficiency in the area of not only social security policy in general and disability policy. This research analyzes cooperation between health and social welfare policy for disabled people by the concepts of ‘institutional complexity’ and ‘institutional logics’. The results are as follows. In the Policy Space, under-development of health and social welfare policy respectively, exclusiveness each other, Introduction and Pervasiveness of ‘Independent Living Model’ and ‘Social Model of Disability’ and Demo Project of Community Care were considered as adding points of institutional complexity. In the Governance Space, unsystematic concepts and relationship between health social welfare at the level of legislations, parallel coexistence between CBR project from public health side and integrated case management system from social welfare are those. In the Time Space, positive feedback is pervasive rather than negative feedback. Thus all these aspects add to the institutional complexity. In relation to institutional logics, it has not been transfered to serious stage yet that which side should take the lead and occupy the centrality between public health and social welfare as the level of cooperation between the two is still very low enough not to evoke coordination and conflicts. This research embodies significant implication as follows; firstly, it analyze cooperation between public health and social welfare policy for disabled people which is not common in the area of academic research. Secondly it apply theoretical frame of new institutionalism in analyzing various aspect surrounding institutional change for the first time. Nonetheless there are some limits as it does not include listening various stakeholders surrounding related policy and starting from the voice of disabled people experiencing both area through the daily life.
