시신경유두부종을 동반한 이글 증후군 사례

국문 초록

목적: 양안 시신경부종이 관찰된 환자에서 경상돌기 비대로 인한 내경정맥 압박(이글 증후군)이 확인된 1예를 보고하고자 한다. 증례요약: 46세 남자가 10일 전부터 양안이 흐리게 보여 내원하였다. 양안 최대교정시력이 1.0, 안저검사상 양안의 시신경부종이 보였다. 뇌 자기공명정맥조영 영상(brain magnetic resonance venography)에서 양측 내경정맥 협착이 보였고 목 컴퓨터단층정맥조영영상(neck computed tomography venography)에서 경상돌기와 제1경추 가로돌기 비대로 인한 양측 내경정맥 협착이 보였다. 이에이글 증후군 진단하 두개내압을 낮추기 위해 아세타졸아마이드 8주간 경구 투여하였으며 양안 시신경부종과 환자의 시각 증상이 소실되었다. 결론: 경상돌기가 비정상적으로 비대해지면 주변 혈관이나 신경을 압박할 수 있으며 내경정맥을 압박하는 경우 두개내압상승으로인한 시신경유두부종을 일으킬 수 있다. 따라서 시신경유두부종 환자의 뇌 자기공명정맥조영 영상에서 내경정맥 협착을 보이는 경우에는 이글 증후군을 감별진단에 포함시켜야 하며, 목 컴퓨터단층정맥조영 영상을 촬영하여 혈관과 주변 뼈 구조물들의 해부학적 위치관계를 확인하는 것이 필요하다.

영문 초록

Purpose: We present a case of Eagle’s syndrome secondary to styloid process enlargement in a patient with papilledema. Case Summary: A 46-year-old male presented with a 10-day history of bilateral blurred vision. Despite a best-corrected visual acuity of 20/20 in both eyes, fundoscopic examination revealed bilateral optic disc swelling. Brain magnetic resonance venography (MRV) demonstrated tortuous and narrowed bilateral internal jugular veins. Neck computed tomography venography (CTV) confirmed bilateral internal jugular vein stenosis secondary to enlargement of styloid and transverse processes of the first cervical vertebra, consistent with Eagle's syndrome. Treatment with oral acetazolamide led to improvement in papilledema and visual symptoms. Conclusions: Styloid process enlargement can lead to compression of adjacent structures, including blood vessels and nerves. In our patient, compression of the internal jugular vein contributed to papilledema. The presence of papilledema and internal jugular vein stenosis on brain MRV should raise suspicion for Eagle's syndrome. Neck CTV is crucial for evaluating the anatomical relationship between vascular structures and bony elements, aiding diagnosis and guiding treatment.
