파장가변광원 전안부 빛간섭단층촬영을 이용한 섬모체해리의 진단과 치료 경과의 판정 3예

국문 초록

목적: 섬모체해리 환자에게 파장가변광원 전안부 빛간섭단층촬영(swept-source anterior segment optical coherence tomography, SOCT)을 이용하여 진단과 치료 및 경과 관찰을 한 3예를 보고하고자 한다. 증례요약: (증례1) 안구 외상 후 발생한 급성기 저안압 11세 환아로 SOCT로 전방각경검사로 발견이 어려웠던 미세한 섬모체해리 틈새를 진단하고 조절마비제 치료 후 시력과 안압, 섬모체해리 모두 호전되었다. (증례2) 낙상 후 발생한 광범위한 섬모체해리가 동반된만성기 저안압 60세 남자 환자로 섬모체해리 간접봉합술을 시행하였고 섬모체해리의 치료 후 호전 여부를 SOCT를 통해 확인할 수있었다. (증례3) 여과포복원술 후 합병된 조절되지 않은 저안압과 전방 허탈로 내원한 67세 환자로 SOCT를 이용하여 섬모체해리를진단하고 조절마비제 점안 후 시력, 안압, 여과포 그리고 섬모체해리의 호전을 확인할 수 있었다. 결론: 파장가변광원 전안부 빛간섭단층촬영은 섬모체해리 틈새의 정도를 잘 식별하고 시각화하여 평가할 수 있으며, 치료 후 경과관찰에서도 유용하게 사용할 수 있었다.

영문 초록

Purpose: To report three cases of cyclodialysis diagnosed and monitored using swept-source anterior segment optical coherence tomography (SOCT). Case Summary: (Case 1) An 11-year-old child with acute hypotony following ocular trauma was diagnosed with subtle cyclodialysis clefts using SOCT, which were difficult to detect on gonioscopy. Visual acuity, intraocular pressure (IOP), and cyclodialysis improved after treatment with cycloplegic agents. (Case 2) A 60-year-old male with chronic hypotony and extensive cyclodialysis following a fall underwent indirect cyclopexy. Post-treatment improvement in cyclodialysis was monitored using SOCT. (Case 3) A 67-year-old patient with uncontrolled hypotony and a collapsed anterior chamber after trabeculectomy revision surgery was diagnosed with cyclodialysis using SOCT. Visual acuity, IOP, filtering bleb, and cyclodialysis improved after treatment with cycloplegic agents. Conclusions: SOCT provides accurate identification and visualization of cyclodialysis clefts, making it useful for evaluating the extent of cyclodialysis and monitoring post-treatment progress.
