국문 초록

연구목적: 이 연구의 목적은 공적해외원조(ODA) 수원국으로서의 한국의 경험을 바탕으로 지역사회 건강불평등 해소를 위한 일차보건의료체계 강화 전략을 탐색하고 개발도상국의 지속가능한 일차보건의료체계를 구축할 수 있는 전략적 인사이트를 제공하는 것이다. 연구의 중요성: 이 연구는 포괄적인 접근에 의해 차별화되었던 한국의 지역사회 보건사업을 통해, 특히 ODA 수원국에서 공여국으로 전환하는 동안 건강격차를 완화하였던 일차보건의료체계 구축과 지역사회병원의 역할에 중점을 두었다. 연구방법론: 1960년대부터 1990년대까지 거제군 및 강화군 (시범)보건사업 등 일차보건의료사업 이니셔티브의 주요 사례에 대한 국내외 학술논문, 보고서, 역사 기록물 345편을 수집하여 질적 체계적 검토 방법론에 준하여 내용분석을 시행하였다. 연구결과: 한국의 8개 지역사회 내 수행된 지속가능한 일차보건의료체계 개발 사례에서 국제 원조, 지역사회 참여, 정부 주도의 통합 전략이 중요한 역할을 하였다. 특히 1970년대 보건 인프라와 인적자원이 절대적으로 부족하였던 상황을 감안할 때 일차보건의료 접근전략에 따른 지역맞춤형 보건의료서비스는 지역사회의 요구를 충족시켰으며, 다양한 ODA 재정을 포함한 세계보건기구의 지속적인 기술 지원은 지역보건의료체계 구축의 안정성을 보장하였다. 결론 및 시사점: 이 연구를 통하여 보건의료서비스의 현지화, 지역사회 참여 촉진, 국제 원조 후 독립적인 운영을 위한 리더십의 중요성을 확인하였다. 결론적으로 개발도상국의 건강 격차를 완화하고 회복력 있고 공평한 일차보건의료체계 구축을 위한 실행 가능한 전략을 제공할 수 있다.

영문 초록

Purpose: This study examines South Korea’s experience as an Official Development Assistance (ODA) recipient, focusing on its efforts to strengthen primary health care (PHC) systems to address health disparities. The study aims to provide strategic insights for developing countries to establish sustainable PHC systems based on South Korea’s experience. Originality: This study is distinct in its comprehensive analysis of South Korea's community health projects, particularly focusing on their role in mitigating health disparities during the country's transition from an ODA recipient to a donor. Methodology: A qualitative systematic review of 345 articles, reports, and historical records was conducted. Key cases of PHC initiatives, such as the Ganghwa Community Health Project and the Geoje Community Development Health Project, were analyzed. Result: The study highlights the critical role of international aid, community participation, and government-led integration in developing sustainable PHC systems. South Korea's tailored health services addressed community needs, while ongoing government support ensured the stability of healthcare infrastructure. Conclusions and Implication: South Korea’s experience emphasizes the importance of localizing healthcare services, fostering community involvement, and preparing for independent operation after aid. The findings provide actionable strategies for developing countries to mitigate health disparities and build resilient, equitable healthcare systems.
