ICF 모델에 근거한 독거노인의 자살생각 예측모형

국문 초록

본 연구의 목적은 WHO의 ICF(International Classification of Functioning, Disability and Health)모델을 기반으로 독거노인의 자살생각을 통합적으로 설명하고 예측할 수 있는 모형을 구축하여 모형의 적합도를 검증하고, 독거노인의 자살생각에 영향을 미치는 요인 간 직⋅간접효과를 파악하고자 함이다. 본 연구를 위해 보건복지부와 한국보건사회연구원에서 실시한 2017년 노인실태조사 데이터를 활용하여 만 65세 이상 독거노인 2,303명을 최종 분석에 사용하였다. 자료분석은 SPSS 21.0과 Amos 22.0 프로그램을 사용하였고, 구조방정식모형을 이용한 경로분석을 실시하였다. 경로분석 결과, 독거노인의 자살생각에 직접적인 영향력이 가장 큰 요인은 우울이었고, 다음으로 경제수준, 연령, 만성질환, 성별 순으로 확인되었다. 자살생각에 간접 영향을 미치는 요인은 사회적 지지 만족도, 주관적 건강상태, 교육수준, 만성질환, 도구적 일상생활수행능력, 연령, 비공식적 지지, 취업여부, 사회참여 순이었다. 본 연구결과를 토대로, 독거노인의 자살을 예방할 수 있는 사회복지 차원의 개입방안들을 제시하였다.

영문 초록

The present study aims to establish a model based on the ICF(International Classification of Functioning, Disability and Health) that can comprehensively explain and predict suicidal ideation among elderly people living alone to test the model’s goodness of fit and examine the direct and indirect effects of factors influencing the suicidal ideation. The present study uses the 2017 Survey on the Living Conditions of Older Persons conducted by the Ministry of Health and Welfare and the Korea Institute for Health and Social Affairs. The data of 2,303 elderly people living alone (65 or older) is examined for a final analysis. SPSS 21.0 and Amos 22.0 software programs are used for data analyses, and structural equation modeling is used to conduct a path analysis. The results were as follows. First, the factor that has the biggest direct effect on suicidal ideation among elderly people living alone has been identified as depression, followed by economic level, age, chronic diseases, and gender. Second, factors that have an indirect effect on suicidal ideation were in the order of satisfaction with social support, subjective health state education level, chronic diseases, instrumental activities of daily living (IADL), age, informal support, employment, and social participation. Based on the results, this study suggested social welfare interventions to prevent suicide of elderly people living alone.
