국문 초록

어떠한 방향의 치아 이동이라 할지라도 교정치료가 마무리된 이후 치아는 원래의 위치로 돌아가려는 주변 조직의 힘을 받게 되므로 유지장치의 사용은 필수적이며 고정식 유지장치는 오랫동안 선호되어 사용해온 유지 장치의 대표적인 방법이다. 자주는 아니더라도 드물게 고정식 장치의 변형으로 인한 원하지 않는 치아의 위치변화 또는 이로 인한 주변 치주조직의 파괴 등 합병증을 발견할 수 있으며, 이러한 고정식 유지장치의 변형은 여러가지 형태로 나타날 수 있고 이러한 포괄적인 합병증을 “Wire syndrome”이라 한다. 아직은 교정학에서 널리 통용되는 용어가 아니므로 이러한 용어를 접하지 못한 교정과 의사도 있을 수 있으나, 어느 정도 기간의 환자 경험을 가진 임상의라면 이러한 유지장치의 변형으로 인한 합병증은 한번쯤 은 접할 수 있는 현상이라 볼 수 있다. 본 논문에서는 이러한 “Wire syndrome”이 임상적으로 어떠한 양상으로 나타날 수 있는지 알아보고, 원인적 요소가 무엇인지 고찰해보고자 한다. 또한 이러한 합병증이 나타났을 때 이에 대한 진단과 해결 방법이 어떤 것이 있을지 고민해보고자 한다.

영문 초록

The use of a retainer is essential after orthodontic treatment because teeth are subjected to the forces of the surrounding tissues that try to restore them to their original positions, regardless of the direction of tooth movement. Fixed retainers have long been preferred as the representative retention method. Although infrequent, complications such as unwanted tooth position changes due to deformation of the fixed appliance or resulting destruction of surrounding periodontal tissues can occur. This deformation of fixed retainers can manifest in various forms, and these comprehensive complications are referred to as "wire syndrome." As this term is not yet widely used in orthodontics, some orthodontists may not be familiar with it. However, complications due to retainer deformation are a phenomenon that can be encountered at least once by experiences clinicians. This paper aimed to investigate the clinical aspects of "wire syndrome,” examine its causative factors, and contemplate diagnostic and treatment methods for these complications.
