횡적 부조화와 교합 간섭에 따른 불안정한 교합을 가진 골격성 II급 부정교합 환자의 교합 재구성 치험례

국문 초록

목적: 불안정한 교합을 가진 비대칭 환자에서 치료적 진단을 바탕으로 한 교정치료를 통해 안정적인 교합을 재구성하고, 심미적인 안모를 얻은 증례를 보고하고자 한다. 증례: 본 29세 남환은 개인 치과의원에서 개방교합과 교합 불안정을 주소로 의뢰되었으며 어금니까지 물려고 하면 턱이 틀어지는 것 같다고 호소하였다. 야간 이갈이 습관 때문에 6개월 마다 양측 교근과 측두근에 보톡스 주사를 맞은 병력이 있었다. 횡적 부조화와 교합 간섭, 전반적인 교모, 중증 치주염에 이환된 골격성 II급 부정교합 및 안면 비대칭으로 진단되었고, 교합 시 좌측으로 편위되는 기능적 변위가 의심되었다. 교합안정장치를 이용한 치료적 진단을 통해 정중선 편위 없이 안정된 하악위를 확립하였다. 이후 miniscrew-assisted rapid palatal expansion (MARPE)를 이용한 횡적 부조화의 개선 및 상악 제2대구치의 압하를 통해 하악골의 auto-rotation을 유도하여 적절한 수직피개와 수평피개를 얻었다. 교정장치 제거 후 고정식, 가철식 유지장치와 더불어 야간 교합안정장치의 장착을 지시하여 이갈이 습관에 따른 불안정한 교합의 재발을 방지하도록 하였다. 결론: 치료적 진단으로 비대칭의 원인을 감별하고, 불안정한 교합에 대한 정밀한 분석과 구체적인 치료 목표 설정을 통해 심미적이고 기능적인 치료 결과를 얻을 수 있다.

영문 초록

Purpose: To report the case of a patient with facial asymmetry and unstable occlusion who underwent orthodontic treatment based on a therapeutic diagnosis to achieve an aesthetic appearance with stable occlusion. Case Report: A 29-year-old man was referred from a local dental clinic for treatment of an open bite and unstable occlusion. The patient complained that his jaw appeared to shift when he attempted to clench his molars. He had a history of bilateral Botox injections in the masseter and temporalis muscles every 6 months for the treatment of bruxism. The patient was diagnosed with skeletal Class II malocclusion, facial asymmetry with transverse discrepancy, occlusal interference, generalized attrition, and severe periodontitis; a functional shift to the left was suspected during occlusion. A therapeutic diagnosis using a stabilization splint established a stable mandibular position without midline displacement. The transverse discrepancy was improved with miniscrew-assisted rapid palatal expansion (MARPE), and intrusion of the maxillary second molars was subsequently performed. This treatment induced auto-rotation of the mandible and achieved adequate overjet and overbite. After the removal of the orthodontic appliance, the patient was provided with a bonded lingual fixed retainer and instructed to wear removable retainers. The patient was also instructed to wear a maxillary stabilization splint at night to prevent recurrence of unstable occlusion due to bruxism. Conclusion: The therapeutic diagnosis allowed us to delineate the causes of the asymmetry. Moreover, the precise analysis of the unstable occlusion with specific treatment goals resulted in improved aesthetic and functional treatment outcomes.
