악교정 수술 후 과발산형 안모 및 입술폐쇄부전을 보이는 환자의 상악 전치열 함입 치험례

국문 초록

목적: 악교정 수술 후 안면 고경 증가와 입술폐쇄부전을 동반한 골격성 II급 관계가 관찰된 환자에서, 생역학적 고찰을 기반으로 전상악 치열 함입을 통해, 부가적인 수술 없이 교정적으로 기능과 심미를 모두 개선한 증례를 소개하고자 한다. 증례: 20세 여성 환자가 안면 비대칭을 주소로 교정 치료 상담을 위해 내원하였다. 임상 및 방사선적 평가를 기반으로, 안면 비대칭을 동반한 골격성 III급 경향의 I급 부정교합으로 진단하였다. IVRO (Intraoral vertical ramus osteotomy)와 genioplasty를 동반한 악교정 수술 후, 환자는 술전 계획과 달리 과발산형 안모 및 입술폐쇄부전과 함께 골격성 II급 관계를 보였고, 하악의 후방 이동이 관찰되었다. 이후 상악 전치열 압하를 동반한 술후 교정치료를 진행하였으며, 치료기간은 총 27개월이 소요되었다. 결론: 악교정 수술 전 시뮬레이션과 다른 방향으로 수술이 시행되어, 기능적 불편감과 부자연스러운 측모가 초래되었다. 교정용 미니 스크류를 이용한 전치열 압하를 통해 기능적 문제와 측모를 개선하였다. 치료 결과가 안정적으로 유지되는지 확인하기 위해 장기적인 관찰을 진행할 예정이다.

영문 초록

Purpose: A patient presented with a skeletal Class II relationship accompanied by an increased facial vertical dimension and lip incompetence following orthognathic surgery. This report introduces a case in which both function and aesthetics were orthodontically improved without additional surgery by intruding the maxillary dentition based on biomechanical considerations. Case Report: A 20-year-old woman visited our clinic for an orthodontic treatment consultation with facial asymmetry as the chief complaint. Based on the clinical and radiographic evaluations, the patient was diagnosed with a skeletal Class I malocclusion with a skeletal Class III tendency, accompanied by facial asymmetry. After orthognathic surgery involving intraoral vertical ramus osteotomy (IVRO) and genioplasty, the patient exhibited a skeletal Class II relationship contrary to the presurgical plan. In addition, a hyperdivergent facial pattern, lip incompetence, and posterior repositioning of the mandible were observed. Subsequently, postoperative orthodontic treatment involving intrusion of the maxillary dentition was conducted, and the total treatment duration was 27 months. Conclusion: Because the surgery was performed in a manner different from the presurgical simulation, it resulted in functional discomfort and an unnatural profile. Both functional problems and the profile were improved by intruding the maxillary dentition using orthodontic mini-screws. Long-term follow-up will be conducted to confirm the stability of the treatment outcomes.
