전신질환 및 측두하악관절 골관절염이 있는 성인 골격성 II급 부정교합 환자에서 골고정성 악궁확장장치를 적용한 비수술 치험례

국문 초록

목적: 본 증례는 전신질환과 턱관절 골관절염, 횡적 부조화 및 큰 수평피개를 보이는 골격성 II급 장안모 환자에서 면밀한 경과관찰 및 개별적인 치료계획 수립을 통한 성공적인 비수술 치료 접근 방법을 보여주었다. 증례: 19세 여성 환자가 측두하악관절 골관절염과 총생을 주소로 내원하였다. 전신적으로 유년형 특발성 관절염, 그레이브스병 및 발작성 상심실성 빈맥 병력이 있었으며, 측두하악관절 골관절염이 있었다. 임상 및 방사선적 평가를 기반으로 측두하악관절 골관절염, 심한 공간 부족 및 횡적부조화를 동반한 골격성 II급 부정교합으로 진단하였으며, 전신질환 및 측두하악관절 골관절염 증상 완화된 이후 교정치료를 시작하였다. 상악에 골고정성 악궁확장장치를 적용하여 횡적 부조화를 해결하였고, 상악 전치의 최대 후방이동 및 하악 구치의 전방이동으로 적절한 교합관계와 심미적인 결과를 달성하였다. 환자는 치료 결과에 만족하며 치료기간은 총 24개월이 소요되었다. 결론: 측두하악관절 골관절염이 있는 환자에서 교정치료 시 치료 전과 치료 중 면밀한 경과 관찰이 중요하다. 또한 치성 보상되어있는 협착된 상악궁에서 골고정성 악궁 확장장치를 통해 효과적인 악궁 확장을 달성하였다.

영문 초록

Purpose: This case report demonstrates a successful nonsurgical treatment approach through careful monitoring and the establishment of an individualized treatment plan for a patient with skeletal Class II malocclusion with a systemic disease, temporomandibular joint osteoarthritis, transverse discrepancy, and significant horizontal overbite. Case Report: A 19-year-old woman presented with temporomandibular joint osteoarthritis and macrognathia. She had a medical history of juvenile idiopathic arthritis, Graves disease, paroxysmal supraventricular tachycardia, and temporomandibular joint osteoarthritis. Based on the clinical and radiographic evaluations, the patient was diagnosed with temporomandibular joint osteoarthritis, severe space deficiency, and skeletal Class II malocclusion with a transverse discrepancy. Orthodontic treatment was initiated after the symptoms of systemic disease and temporomandibular joint osteoarthritis were alleviated. A bone-anchored maxillary expansion device was applied to address the transverse discrepancy, and appropriate occlusal relationships and aesthetic results were achieved through maximum retraction of the maxillary incisors and anterior movement of the mandibular molars. The patient was satisfied with the outcomes of the treatment, which took 24 months to complete. Conclusion: Careful monitoring before and during orthodontic treatment is crucial for patients with temporomandibular joint osteoarthritis. Effective arch expansion was achieved using a bone-anchored maxillary expansion device in a constricted maxillary arch with dental compensation.
