외국인 유학생의 의료보장 및 의료서비스 인식과 경험에 관한 혼합연구: K대학 사례를 중심으로

국문 초록

본 연구는 외국인 유학생의 건강권 보장 차원에서 추진된 국민건강보험의 의무 적용 후 이에 대한 외국인 유학생의 인식과 경험을 통해 그 의의와 한계를 탐색하는데 목적을 지니고 있다. 이를 위해 외국인 유학생의 의료보장 현황과 의료서비스 이용 경험에 관한 통합적인 이해를 도모하고자 양적 연구와 질적 연구를 고려한 혼합연구 방법론을 실시하였다. 연구 결과, 외국인 유학생들은 국민건강보험의 의무 적용에 대해 87.4%가 긍정적으로 수용하고 있었으나, 보험료를 정확하게 알지 못하는 수준이 66.8%, 현재 미가입 상태가 39.5%, 연체 경험이 52.6%, 보험료 부담이 52.1% 등으로 전반적인 제도 적용 수준이 낮았고, 적정 보험료는 2만원~4만원이라고 응답하였다. 또한 외국인 유학생의 경우 지난 1년간 병원에 한번도 간 적이 없는 사람이 44.2%이고, 아프거나 다칠 경우에 31.6%만이 병원에 간다고 응답해 의료서비스 이용이 저조하였으며, 치료비 부담, 한국어 미숙, 병원에 대한 이해 부족 등이 의료서비스 접근을 어렵게 하는 이유로 꼽았다. 또한 양적 연구 결과의 맥락적 이해를 돕기 위해 실시한 질적 연구에서는 ‘출신국에서 의료서비스 선경험 부족’, ‘건강보험제도가 뭔 줄도 모르고 가입자가 되어 있음’, ‘이용하지 않은 의료서비스/그러다가 납부 중단’, ‘예고없이 찾아오는 사고로 의료서비스 경험’, ‘건강보험 중요성 실감’, ‘여전히 두렵고 어려운 병원 이용’, ‘건강보험 제도와 의료서비스에 대한 체험 교육 필요’ 등 7가지 범주가 도출되었다. 이러한 양적 연구와 질적 연구 결과를 토대로 외국인 유학생의 의료보장 및 의료서비스 인식 및 이용 개선을 위한 정책적, 실천적 방안을 제언하였다.

영문 초록

The purpose of this study is to explore the significance and limitations of the mandatory implementation of National Health Insurance, which was promoted to ensure the right to health of international students, through the perception and experience of international students. To this end, a mixed research methodology that considered quantitative and qualitative research was conducted to promote an integrated understanding of the current status of medical insurance and the experience of using medical services of international students. The research results showed that 87.4% of international students positively accepted the mandatory application of national health insurance, but 66.8% did not know the exact insurance premium, 39.5% were currently not enrolled, 52.6% had overdue payments, and 52.1% were concerned about the insurance premium, showing a low level of understanding overall. They responded that the appropriate insurance premium was between 20,000 and 40,000 won. In addition, 44.2% of international students had not been to a hospital in the past year, and only 31.6% responded that they went to a hospital when sick or injured, showing low utilization of medical services. They cited the burden of medical expenses, lack of Korean language proficiency, and lack of understanding of hospitals as reasons for hindering access to medical services. In addition, a qualitative study conducted to help understand the context of the quantitative research results resulted in seven categories: ‘lack of prior experience with medical services in the country of origin’, ‘became a subscriber without even knowing that the health insurance system was far away’, ‘medical services not used/then payment stopped’, ‘experienced medical services due to unexpected accidents’, ‘realized the importance of health insurance’, ‘still fearful and difficult to use hospitals’, and ‘need for experiential education on the health insurance system and medical services’. Based on these quantitative and qualitative research results, policy and practical measures were suggested to improve the awareness and use of medical insurance and medical services by international students.
