강박장애의 인지행동치료 방안

국문 초록

강박장애의 인지행동 모델은 정상적인 침투사고가 강박사고로 변질되는 과정을 잘 설명하고 있다. 즉 과도한 책임감 지각이나 사고에 대한 파국적 오해석 등이 침투사고의 현저성을 증가시키고 중화행동을 더욱 강화시킴으로써 강박장애 메커니즘을 유지시킨다는 것이다. 강박장애의 인지행동치료는 노출 및 반응방지 기법을 중심으로 한 행동적 전략을 통하여 강박증상을 유지시키는 중화행동 및 회피행동의 고리를 끊고, 더불어 인지적 전략을 사용하여 환자들의 왜곡된 평가 및 비합리적 신념을 수정하고 보다 융통성 있고 현실적인 평가로 대체시킨다. 본 연구의 결과는 다음과 같다. 첫째, 강박장애를 가진 내담자의 침투사고가 강박사고로 변하게 하는데에 가속화시키는 영향을 미친다. 둘째, 중화행동을 수행하였을 때 강박장애를 가진 내담자의 행동은 부적인 영향을 미치는 것으로 결과가 보여졌다. 셋째, 인지행동치료는 강박장애를 가진 내담자의 행동에 영향을 미치는 것으로 결론을 낼 수 있다. 자신의 행동에 대해 정확하게 인지하고, 교정해야할 비합리적 사고를 합리적 사고로 전화시켜 받아들이게 되는 순간부터 강박장애를 조금씩 교정해나갈 수 있게 된다. 본 연구를 통해 강박증을 이해하는데 도움이 되며 상담을 이어나가는 방향을 위한 기초자료로 활용될 수 있으리라 사료된다.

영문 초록

The cognitive behavioral model of obsessive-compulsive disorder explains well the process by which normal infiltration thinking is transformed into obsessive-compulsive thinking. In other words, excessive perception of responsibility or catastrophic misunderstanding of accidents increases the salience of infiltration accidents and further strengthens neutralization behavior, thereby maintaining the obsessive-compulsive disorder mechanism. Error evaluation of infiltration accidents and dysfunctional beliefs that influence them include excessive importance of thinking (convergence of thinking behaviors), excessive responsibility, the importance of accident control, fear of uncertainty, perfectionism, and excessive evaluation of threats. Neutralization behavior is an intentional control method that is attempted to remove and prevent the discomfort of infiltration thinking and change the content of the accident itself, and recent studies have reported that these cognitive evaluation and control methods vary depending on the type of infiltration accident. Cognitive behavioral therapy for obsessive-compulsive disorder breaks the cycle of neutralization and avoidance behaviors that maintain obsessive-compulsive symptoms through behavioral strategies centered on exposure and response prevention techniques, and uses cognitive strategies to correct distorted and irrational beliefs of patients and replace them with more flexible and realistic evaluations. Finally, the possibility of recurrence and points to consider at this time were mentioned. It is believed that this study will help you understand obsessive-compulsive disorder and can be used as basic data for the direction of continuing counseling.
