Background: To investigate the effect of facial botulinum toxin (BTX) injection on the patients with facial synkinesis. Materials and Methods: Medical records and facial palsy questionnaire of the patients with facial nerve paralysis who received BTX injection at Seoul St. Mary’s Hospital by one physician from 2014 to 2021 were retrospectively reviewed. A total of 29 patients were divided into two groups; 15 patients with facial synkinesis who received facial BTX injection on both synkinetic and contralateral sides of facial palsy (bilateral botulinum toxin group [BG]) and 14 patients without facial synkinesis who received BTX injection only on the contralateral side of the facial palsy (contralateral botulinum toxin group [CG]). Results: Complete control of the synkinetic motion in the BG and more symmetric facial expression in both groups were observed for about 4 months after BTX injection. No patient showed any complication. Time to initial facial BTX injection from the onset of the facial palsy was significantly longer in the BG than the CG (P < 0.05). Average number of facial BTX injection and follow-up duration were both significantly greater in the BG than in the CG (P < 0.05). Patients in both groups showed significant improvement in scores of the questionnaire after BTX injection (P < 0.05). The proportion of the patients showing complete recovery was significantly higher in the CG (57.1%) than the BG (26.7%). Conclusions: Facial BTX injection seems to be safe and effective for both cosmetic and functional results in patients with facial nerve paralysis.
