Background: Endoscopic endonasal approach (EEA) for tuberculum sellae meningioma (TSM) has shown favorable visual outcomes. However, EEA cannot remove the laterally extended tumor. In this study, we aim to analyze the clinical outcomes of combined transcranial approach (TCA) and EEA for TSM with optic canal involvement (OCI) and lateral extension. Materials and Methods: We retrospectively analyzed 16 patients who underwent surgery for TSM with combined TCA and EEA from March 2010 to February 2023. We analyzed the surgical parameters, clinical outcomes, and visual outcomes of the patients. Results: Gross total resection (GTR) was achieved in 12 patients (75%). The three patients (18.8%) who had residual tumors received Gamma Knife surgery, and one patient (6.3%) underwent regular follow-up. One patient (6.3%) received radiation therapy after GTR for World Health Organization grade 2 meningioma. Postoperative cerebrospinal fluid leakage and meningitis occurred in 1 patient (6.3%) each. One patient (6.3%) experienced non-convulsive status epilepticus postoperatively, and transient syndrome of inappropriate antidiuresis and diabetes insipidus were present in 3 patients (18.8%) each. There was no intracranial hemorrhage or cerebral ischemia after the surgery. With the exception of one patient, 15 patients (93.8%) showed favorable visual outcomes after surgery. Conclusions: Combined TCA and EEA are beneficial for the removal of tumors and improvement of visual symptoms in TSM with lateral extension and OCI.
