배우자 돌봄과 주거환경 특성이 고령여성의 생활만족도에 미치는 영향 연구

국문 초록

본 연구의 목적은 노년기 돌봄과 일상생활이 이루어지는 공간인 ‘주거환경’에 주목하여 고령여성의 배우자 돌봄과 주거환경 현황을 살펴보고, 고령여성의 생활만족도 제고를 위한 정책적시사점을 도출하는 것이다. 분석자료는 『초고령사회 대비 고령여성의 삶의 질 현황과 정책과제: 돌봄과 주거를 중심으로』(최인희 외, 2022)에서 수집한 설문조사 원자료(수도권에 거주하며 일상생활수행능력에 제한이 있는 배우자를 정기적으로 돌보는 60세 이상 고령여성 대상)를 활용하였고, 기술통계 분석과 집단차이 분석, 회귀방정식 모형을 통해 고령여성의 배우자 돌봄현황과주거환경 특성을 살펴보고, 이러한 특성이 고령여성의 생활만족도에 미치는 영향을 파악하였다. 연구결과, 본 조사에 참여한 고령여성은 배우자를 1주 평균 39.5시간가량 돌보는 것으로 나타났으며, 배우자 돌봄기간은 2년 미만인 고령여성이 전체 응답자의 약 3/5으로 나타났다. 6개영역의 돌봄활동별 부담을 측정한 돌봄부담 총점은 17.7점으로 나타났고, 세부 영역 중 개인위생 지원활동, 외출 시 동행 부담이 상대적으로 큰 것으로 나타났다. 한편, 배우자를 돌보는70세 이상 여성노인은 70세 미만 고령여성과 비교할 때 만성질환 수가 더 많고, 돌봄부담(총점, 식사지원, 가사지원, 개인위생 지원)을 상대적으로 더 크게 느끼고, 생활만족도가 더 낮은것으로 나타났다. 배우자 돌봄과 주거환경 특성이 고령여성의 생활만족도에 미치는 영향을분석한 결과, 고령여성의 주관적 건강상태, 돌보고 있는 배우자의 ADL/IADL 제한 정도, 돌봄부담, 주당 평균 공적 돌봄서비스 이용시간이 유의미한 변인으로 나타났으며, p<.10 수준에서주택 출입 편의성과 채광, 냉방, 난방 등의 양호성이 유의미한 변인으로 나타났다. 이러한 연구결과를 고려할 때, 노노케어 가구를 대상으로 한 생활지원 및 외출지원 서비스, 공적 노인돌봄서비스 확대와 함께 주거환경 개선사업 및 다양한 주거ㆍ돌봄 연계 시범사업 추진 등이 확대될필요가 있을 것으로 보인다.

영문 초록

The purpose of this study was to examine characteristics of elderly women’s spousal caregiving and of the residential environments in which their caregiving and daily life take place in order to analyze their effects on life satisfaction and draw policy implications for enhancing life satisfaction among elderly women who take care of a dependent spouse. Using survey data(data subject: women aged 60 or older who lived in the Seoul metropolitan area and took care of their spouses with limitations in activities of daily living and instrumental activities of daily living) from the study “Current status of elderly women’s quality of life and policy tasks to support it in preparing for a super-aged society: Care and housing” (Choi et al., 2022), descriptive statistics and multivariate models were obtained using software from SPSS Statistics. The results indicate that the elderly women participating in this survey provided their spouses with an average of 39.5 hours of care work per week. Approximately 60% of respondents reported a caregiving period of less than two years. The total burden score for caregiving, measured across six domains, was 17.7. Higher levels of burden were associated with assistance with personal hygiene and accompaniment during outings. Additionally, elderly women over the age of 70 who were caring for spouses reported more chronic health conditions, a higher caregiving burden (i.e., total score, burden for meal support, household chores, and personal hygiene assistance), and lower life satisfaction compared to those under 70. In analyzing the impact of spousal caregiving and residential environment characteristics on life satisfaction, subjective health status, the ADL/IADL limitations of the spouse being cared for, caregiving burden, and average weekly hours of public care services used were all significant predictors for life satisfaction among the elderly women taking care of a dependent spouse. Additionally, at the p<.10 significance level, the ease of access to their housing and the adequacy of lighting, cooling, and heating were also identified as significant predictors. Based on these findings, there is a need to expand household support and outing assistance services for elderly households engaged in elder care, increase public elderly care services (e.g., long-term care services, customized care services for older adults), improve residential environments, and promote various pilot programs linking housing with care services.
