양안의 적도부 및 뒤섬모체소대약화가 관찰된 백내장 증례보고

국문 초록

목적: 외상이나 수술의 과거력 없이 양안 적도부 및 뒤섬모체소대약화가 수술 중에 관찰된 증례를 보고하고자 한다. 증례요약: 73세 여자가 양안 시력저하로 내원하였으며, 양안 최대교정시력 0.5로 측정되었다. 세극등검사에서 양안의 백내장이 관찰되어 수술을 시행하였다. 좌안 수술 중, 전낭절개 시에는 섬모체소대의 약화가 저명하지 않았으나, 수정체유화술 중 적도부 및 뒤섬모체소대 손상으로 상핵과 피질 제거 시 수정체낭의 적도부와 후낭이 계속 유화기 첨단부로 이동하여 수술 진행이 어려웠다. 결국 수술중 후낭이 파열되어, 앞유리체절제술 시행 후 삼체형 인공수정체를 섬모체고랑에 삽입하였다. 2주 뒤 우안 수술 중 좌안과 동일한소견이 발견되어 상핵과 피질의 후낭과의 분리에 어려움이 있었으나, 중간에 관류병 높이를 올리고, 점탄물질을 추가하며, 양손 관류/ 흡인기로 남은 수정체 조각을 제거하여 후낭파열과 앞유리체절제술 없이 성공적으로 수술을 마무리하였다. 결론: 외상이나 수술의 과거력 없이 적도부 및 뒤섬모체소대약화가 관찰될 경우, 성공적인 백내장수술을 위해서는 관류병 높이를 높이고, 점탄물질을 추가하면서, 양손 관류/흡인기를 사용하는 것이 좋겠다.

영문 초록

Purpose: We present a case of equatorial and posterior zonular weakness observed during cataract surgery in both eyes with no prior history of trauma or surgery. Case summary: A 73-year-old woman visited out clinic and reported decreased visual acuity. She was found to have a best corrected visual acuity of 0.5 in both eyes. Cataracts were noted during a slit lamp examination and cataract surgery was subsequently performed. During the left eye surgery, although there was no evident weakening during the anterior capsulotomy, the equatorial and posterior zonules were damaged during phacoemulsification. This led to consistent aspiration of the equatorial and posterior capsule (PC) into the phaco-handpiece tip during epinucleus and cortex removal. The PC eventually ruptured necessitating anterior vitrectomy and a 3-piece intraocular lens insertion into the ciliary sulcus. Two weeks later, the right eye surgery encountered similar challenges in removing the epinucleus and cortex. The surgery was completed successfully by elevating the irrigation bottle, frequently injecting an ophthalmic viscoelastic device (OVD), and removing the remaining lens material using a bimanual irrigator/aspirator. Conclusions: This case highlights that equatorial and posterior zonular weakness can occur in the absence of trauma or prior to surgery. To manage such cases, it is advisable to elevate the irrigation bottle height, continuously inject OVD during surgery, and use a bimanual irrigator/aspirator.
