간헐외사시에서 술 후 3년 이상 경과 관찰하여 사시각이 안정된 환자들의 재내원 사유 및 임상 양상

국문 초록

목적: 간헐외사시 환자에서 양안 외직근 후전술 시행 3년 이상 이후 자의로 외래에 재내원한 환자들의 사유와 임상 양상을 분석하였다. 대상과 방법: 간헐외사시 진단하 양안 외직근 후전술 시행 후 3년 이상 외래 경과 관찰 이후 재내원한 환자들의 의무기록을 후향적으로 분석하였다. 결과: 총 32명의 환자 중, 자의로 병원을 내원한 이유는 재발 의심이 21명(65.6%)으로 가장 빈도가 높았다. 재내원 사유를 재발이의심되는 군(exodrift)과 그 외(no exodrift)인 군으로 나눠서 비교하였을 때 재내원 시의 사시각은 exodrift군에서 원거리, 근거리로모두 no exodrift군에 비해 더 높은 것으로 나타났다. 전체 환자군과 exodrift군에서 재내원 시 마지막 방문보다 원거리, 근거리 사시각모두 증가하였다. 근거리 입체시는 모든 환자군에서 마지막 방문과 재내원 시 차이가 없었고, no exodrift군의 좌안만 제외하고는모든 군에서 마지막 방문보다 재내원 시 근시도수가 증가하였다. 재내원 이후 지속적인 추적 관찰을 한 환자 중 재수술을 시행한1명을 제외한 11명의 환자의 사시각은 굴절이상 교정 이후 1년 동안 안정되었다. 결론: 간헐외사시 수술 3년 이후 교정시력 저하로 인한 융합눈모음의 저하, 그로 인한 사시각의 불안정성이 흔히 발생하나, 안경 교정이후에 더 진행 없이 사시각이 안정되었다. 따라서, 경과 관찰이 끝날 시점에 이러한 점을 환자 및 보호자에게 추가적으로 설명하는것이 중요하겠다.

영문 초록

Purpose: To analyze the chief complaints and clinical characteristics of patients with intermittent exotropia who voluntarily revisited our clinic more than 3 years after stabilization of bilateral lateral rectus recession. Methods: This study retrospectively reviewed the medical records of patients diagnosed with intermittent exotropia who underwent bilateral lateral rectus recession and revisited the outpatient clinic after more than 3 years of follow-up. Results: Thirty-two patients were included. The most common reason for voluntary revisit was suspicion of recurrence (n = 21, 65.6%). Reasons for revisit were divided into suspicion of recurrence (exodrift group) and others (no exodrift group). At the revisits, both the distance and near deviation angles of the exodrift group were larger than those of the no exodrift group. For all patients and the exodrift group, both the distance and near deviation angles were higher at the revisits than at the last follow-up visits. For both groups, the near stereoacuity did not differ between the last follow-up visit and the revisit but the spherical equivalent was more myopic at the revisit than the last follow-up visit in all eyes except the left eyes of the no exodrift group. The deviation angles of 11 patients who underwent continuous follow-up after the revisits did not significantly change over 1 year after full correction of the refractive error, with the exception of 1 who underwent re-operation. Conclusions: When patients stabilize after intermittent exotropia surgery, the decrease in the corrected visual acuity triggers fusional convergence deterioration and, in turn, unstable strabismus. However, after full correction of the refractive error, the deviation angle stabilizes without further progression. It is important to explain this to patients/guardians at the end of initial follow-up.
