노인 여성의 요실금과 삶의 만족도의 관련성

국문 초록

요실금은 노인 여성에게 흔히 발생하는 건강 문제로, 삶의 만족도에 중대한 영향을 미칠 수 있다. 본 연구는 요실금이 노인 여성의 삶의 만족도와 그 하위 영역인 건강상태 만족도, 경제상태 만족도, 배우자 및 자녀와의 관계 만족도, 전반적인 삶의 질 만족도의 관련성을 파악하는 것을 목적으로 한다. 고령화연구패널조사(KLoSA) 제9차년도(2022년) 데이터를 활용하여, 65세 이상 노인 여성 2,605명을 연구 대상으로 하였다. 요실금 유무에 따른 인구사회학적, 건강 관련, 사회참여 요인의 차이는 카이제곱검정과 t-test로 분석하였고, 대상자의 특성을 통제하여 다중 회귀분석을 통해 요실금 유무에 따른 삶의 만족도의 차이를 분석하였다. 또한, 위계적 회귀분석을 통해 요실금과 삶의 만족도 간의 관련성을 분석하였다. 연구 결과 요실금이 있는 노인여성은 요실금이 없는 여성에 비해 전체 삶의 만족도와 하위 영역인 건강상태, 경제상태, 전반적인 삶의 질 만족도가 유의하게 낮았다. 그러나 배우자 및 자녀와의 관계 만족도는 차이가 없었다. 삶의 만족도 관련 요인을 분석한 결과, 요실금은 건강상태, 경제상태, 전반적인 삶의 질만족도와 유의한 연관성을 보였다. 위계적 회귀분석에서 사회참여 변수를 추가했을 때, 요실금과 삶의 만족도 간의 부정적인 관련성이 완화되었다. 결론적으로 사회참여는 요실금으로 인한 노인 여성의 삶의 만족도 저하를 완화하는 역할을 할 수 있다. 따라서 노인 여성의 삶의 만족도를 향상시키기 위해 요실금 관리와 사회참여 증진이 필요하다.

영문 초록

Urinary incontinence is a common health issue among elderly women and can significantly affect their life satisfaction. This study aims to investigate the relationship between urinary incontinence and life satisfaction in elderly women, as well as its association with subdomains of life satisfaction, including satisfaction with health status, economic status, relationships with spouses and children, and overall quality of life. From the 9th wave of the 2022 Korean Longitudinal Study of Aging (KLoSA), 2,605 women aged 65 and older were included in the analysis. Differences in sociodemographic, health-related, and social participation characteristics by urinary incontinence status were analyzed using chi-square tests and t-tests. Multiple regression analysis was conducted to examine differences in life satisfaction based on the urinary incontinence status, while hierarchical regression analysis was used to explore the relationship between urinary incontinence and life satisfaction. The study results indicate that elderly women with urinary incontinence had significantly lower overall life satisfaction and satisfaction in subdomains such as health status, economic status, and overall quality of life compared to those without urinary incontinence. However, no difference was found in satisfaction with relationships with spouses and children. Urinary incontinence was a predictor of low overall life satisfaction as well as low satisfaction with health status, economic status, and overall quality of life. In hierarchical regression analysis, the negative relationship between urinary incontinence and life satisfaction was mitigated when characteristics of social participation were added. In conclusion, social participation can play a role in alleviating the decline in life satisfaction among elderly women with urinary incontinence. Therefore, to improve life satisfaction in elderly women, it is necessary to enhance both urinary incontinence management and social participation.
