노인 저장행위에 대한 메타리뷰: 개입과 영향요인

국문 초록

본 연구의 목적은 경험적 연구를 바탕으로 수행된 국외의 체계적 문헌고찰에 대한 메타리뷰를 통해 저장행위 문제에 개입하기 위한 실천 기술과 영향 요인을 규명하는 것이다. 기존 체계적 문헌고찰 연구를 대상으로 연구결과를 요약하여 제시하는 연구방법인 메타리뷰를 수행하였으며, 이를 위해 세 가지 온라인 데이터베이스(PubMed, SCOPUS, Web of Science)에서 다음의 키워드를 통해 문헌을 검색하였다: “hoarding” OR “self-neglect” OR “squalor” AND “older adults” AND “systematic review.” 연구 결과는이야기 통합법을 통해 도출되었다. 최초 검색을 통해 총 102편의 문헌이 검색되었고 선별 과정을 거쳐 최종 네 편의 문헌을 분석하였다. 분석결과 저장행위에 대한 효과적 개입방법은 인지행동치료로 나타났다. 개입에 긍정적 영향을 미치는 요인은1) 매뉴얼을 따르는 인지행동치료; 2) 표본의 인구사회학적 특성(성별, 연령); 3) 개별 및 집단 치료, 치료회기, 가정방문 횟수; 4) 약물치료였다. 반면 개입에 부정적 영향을 미치는 요인은 1) 적은 표본 수; 2) 높은중도 탈락률; 3) 개입의 초점 부족으로 나타났다. 이러한 결과를 바탕으로 본 연구는 실천현장에서 활용 가능한 효과적 실천 방법과 함의를 제시하였으며, 특히 개입에 부정적 영향을 미치는 요인을 줄이기 위한 방안과실천가의 역량강화를 위한 교육체계 구축을 제언하였다.

영문 초록

This study sought to identify practical methods and influencing factors for intervening in hoarding behavior. To achieve this, a meta-review was conducted. A meta-review is a research methodology that summarizes and presents findings from existing systematic reviews. An electronic search was conducted in PubMed, SCOPUS, and Web of Science, using the following search phrase: “hoarding” OR “self-neglect” OR “squalor” AND “older adults” AND “systematic review.” A narrative synthesis was conducted to identify effective interventions and influncing factors. The initial search yielded 102 studies. After the screening processes, four studies were eligible for review. The analysis revealed that cognitive-behavioral therapy emerged as an effective intervention method for hoarding behavior. From the data synthesis, four factors that positively influence the intervention emerged: 1) adherence to manualized cognitive-behavioral therapy; 2) demographic characteristics (gender, age); 3) individual and group therapy, treatment duration, and frequency of home visits; and 4) pharmacological treatment. Conversely, factors that negatively affected the intervention included: 1) a small sample size; 2) higher dropout rates; and 3) a lack of focus on intervention. Based on the results, this study provides an effective intervention method and implications applicable in social work settings. Also, it proposes strategies to mitigate factors that negatively impact interventions and suggests the establishment of an educational system to enhance practitioners’ capabilities.
