국문 초록

연구목적: 본 연구는 뇌성마비 환자의 호흡 기능 항목들 가운데 노력성 폐활량, 1초간 노력성 날숨량, 최대 들숨 압력과 최대 날숨 압력이 기능적 독립성을 평가하기 위한 도구인 FIM score와의 상관성을 살펴보고, 호흡 기능이 기능적 독립성에 어떠한 영향을 미치는지 알아보기 위해 실시되었다. 연구방법: 총 14명의 뇌성마비 환자가 연구에 참여하였다. 폐 기능 검사인 노력성 폐활량과 1초간 노력성 날숨량을 측정하였다. 호흡 근력 검사인 최대 날숨 압력과 최대 들숨 압력을 측정하였다. 기능적 독립성 수준을 알기 위해 FIM score를 측정하였다. 폐 기능 검사와 호흡 근력 검사가 기능적 독립성 수준과 어떠한 상관성이 있는지 알아보기 위해 상관관계 분석과 다중회귀 분석을 통하여 살펴보았다. 연구결과: 최대 날숨 압력은 Self care 항목 가운데 Eating과 Grooming, Mobility 항목, Locomotion 항목, 그리고 총 FIM score와 유의한 상관관계가 있었다. 최대 들숨 압력은 Self care 항목 가운데 Eating과 Grooming, Locomotion 항목 가운데 Walk/Wheel chair, Communication 항목 그리고 총 FIM score와 유의한 상관관계가 있었다. 최대 날숨 압력의 증가가 FIM score의 신체 영역에 양의 영향력을 미치는 것으로 나타났다. 결론: 최대 날숨 압력과 최대 들숨 압력은 FIM score와 상당히 여러 항목에서 유의한 상관관계가 있었고, 더 나아가 최대 날숨 압력이 높을수록 기능적 독립성 수준의 향상됨을 알 수 있었다.

영문 초록

Purpose: The purpose of this study was to investigate the relationship between respiratory function and functional independence measure in children with cerebral palsy. Method: 14 subjects who had cerebral palsy participated in this study and participants were assessed and analysed using respiratory function test(forced vital capacity, forced expiratory volume at one second, maximal expiratory pressure and maximal inspiratory pressure) and functional independence measurement score(FIM score). We used pearson s correlation coefficient and regression analysis to know the relationship between respiratory function and functional independence measure score. Results: Maximal expiratory pressure significantly correlated with eating, grooming, bathing, dressing-upper body, dressing-lower body and toilet of self care, bed/chair/wheelchair, toilet and tub/shower of mobility, walk/wheelchair and stairs of locomotion and total FIM score(p<.05). Maximal inspiratory pressure significantly correlated with eating and grooming of self care, walk/wheelchair of locomotion, comprehension and expression of communication and total FIM score(p<.05). Maximum expiratory pressure had a positive effect on the functional independence measurement(p<.05). Conclusion: Maximal expiratory pressure and maximal inspiratory pressure significantly correlated with functional independence measurement. Furthermore, we could know that the higher maximal expiratory pressure is more the ability to perform functional independent measure improves. Our findings suggest that the respiratory function, specially maximal expiratory pressure and maximal inspiratory pressure, may be beneficial for improving their functional independence ability in children with cerebral palsy.
