내부기관 장애인의 지역사회환경 인식이 사회참여에 미치는 영향

국문 초록

본 연구의 목적은 내부기관 장애인의 사회참여 수준을 파악하고, 지역사회환경 인식이 내부기관 장애인의 사회참여에 미치는 영향을 검증하는 것이다. 이를 위해 장애인복지법 상 내부기관 장애인으로 분류되는 신장, 심장, 호흡기, 간, 장루요루, 간질장애인 343명으로부터 자료를 수집하였고, 사회참여를 측정하기 위해 ICF에 기반한 WHODAS 2.0을 사용하였다. 분석결과, WHODAS 2.0 총점은 29.4±19.9점이었고, 응답자의 89.8%가 사회참여 제약을 경험하였고, 특히 응답자의 15.5%는 중증(50-95점) 이상의 상당한 활동 제한 및 참여 제약을 겪은 것으로 나타났다. 사회참여의 하위영역 중제약을 경험한 응답자 비율은 ‘사회활동참여’(89.8%), ‘타인과 어울리기’(80.5%), ‘돌아다니기’(75.5%) 순으로 높았고, 장애유형 중 심장, 간, 호흡기장애인이 사회참여시 더 큰 어려움이 경험하는 것으로 나타났다. 또한 지역사회환경 인식이 사회참여에 미치는 영향을 분석한 결과, 지역사회환경 인식은 개인 및 장애 특성을 통제한 후에도 내부기관 장애인의 사회참여에 유의한 영향을 미치는 것으로 나타났다. 이러한 연구결과를 바탕으로 내부기관 장애인의 사회참여를 위한 지원 확대를 제안하였다.

영문 초록

This study aimed to investigate the level of social participation among people with disability of internal organs using World Health Organization Disability Assessment Schedule (WHODAS-II) and examine the effect of environmental factors on social participation. Three hundred forty three people with disabilities from six disability types (renal function impairment, heart function impairment, liver function impairment, respiratory function impairment, intestinal and urinary tract function impairment, and epilepsy) participated in the study. Social participation was measured with WHODAS-II, which is a generic assessment instrument developed by WHO. The mean of Social participation assessed by WHODAS II was 29.4±19.9. When social participation was reported according to the ICF severity ranges (No problem, 0-4; Mild disability, 5-24; Moderate disability, 25-49; Severe disability, 50-95; Extreme disability, 50-100), it was found that 89.8% of respondents had problems in social participation, and 15.5% had severe difficulties. In the multiple regression analysis, results showed that total and all aspect of social participation except understanding and communicating was directly influenced by perceived community environment, after controlling for types of disabilities and perceived health status. Level of social participation, as measured by WHODAS II, was associated with perceived community environment. An environmental support to improve social participation is important for people with disability of internal organs.
