본 연구의 목적은 스트레스-취약성 이론과 스트레스 대처 이론을 기반으로 노인의 복합만성질환이 우울 수준에 미치는 영향을 분석하고, 그 관계에서 영양상태와 인지기능의 다중가산조절효과를 검증하는 것이다. 이를 통해 노인의 신체적⋅정신적 건강 증진을 위한 통합적 정책 및 개입 전략 마련에 기초자료를 제공하고자 하였다. 본 연구는 2023년 노인실태조사 데이터를 활용하였으며, 우울증을 진단받지 않고 본인 응답을 한 65세 이상 고령자 8,558명의 자료를 분석자료로 사용하였다. 복합만성 질환을 독립변수로, 우울 수준을 종속변수로, 영양상태와 인지기능을 조절변수로 설정하여 SPSS 27.0과 Process macro Model 2를 활용해 분석하였다. 연구 결과, 복합만성질환은 노인의 우울 수준에 유의미한 정적 영향을 미치는 것으로 나타났다. 또한 영양상태 불량은 우울 수준에 정적 영향을, 높은 인지기능은 부적 영향을 미치는 것으로 확인되었으며, 복합만성질환과 우울 수준 간의 관계는 영양상태와 인지기능에 의해 유의미하게 조절되는 것으로 나타났다. 특히, 영양상태가 불량하고 인지 기능이 저하된 노인 집단에서 복합만성질환이 우울 수준에 미치는 영향이 가장 크게 나타났다. 이러한 연구 결과를 바탕으로 노인의 복합적 건강 문제를 예방하고 관리하기 위해 영양개선 및 인지기능유지 프로그램을 포함한 다차원적 통합 지원정책의 필요성을 제언하였다.

영문 초록

This study investigates the impact of multimorbidity on depressive symptoms among older adults within the framework of the stress-vulnerability model and stress-coping theory. It examines the multiple additive moderating effects of nutritional status and cognitive function. It aims to provide foundational evidence for developing integrated policies and interventions promoting physical and mental health in aging populations. Drawing on data from the 2023 National Survey on the Living Conditions of Older Persons in Korea, the study analyzes responses from 8,558 self-reported cases of adults aged 65 years and above, excluding those with diagnosed depression. The research design specifies multimorbidity as the independent variable, depressive symptoms as the dependent variable, and nutritional status and cognitive function as moderating variables. Analyses were performed using SPSS 27.0 and PROCESS Macro Model 2. The findings reveal that multimorbidity significantly increases depressive symptoms among older adults. Poor nutritional status positively correlates with depressive symptoms, while cognitive function shows a negative association. Furthermore, nutritional status and cognitive function jointly moderate the relationship between multimorbidity and depressive symptoms. Specifically, older adults with poorer nutritional status and lower cognitive function experience a more pronounced negative impact of multimorbidity on depressive symptoms. These results underscore the urgent need for multidimensional and integrated support strategies that simultaneously address older adults' physical, nutritional, and cognitive health to improve their mental well-being in an aging society.
